Job Summary
Florida Elder Law & Legacy Planning is seeking a compassionate and experienced Elder Care Navigator to join our growing Life Care Planning Team.

This role is ideal for a medical social worker, nurse or care coordinator who is passionate about helping seniors and their families navigate long term care, healthcare systems, and aging-related challenges.

This position works directly with a Board Certified Elder Law Attorney and key staff to support clients through care planning, advocacy, and Medicaid-related coordination. The Elder Care Navigator serves as an advocate and experienced elder care professional on the legal team. They further provide families with the knowledge and assistance they need regarding their care.

The scope of responsibilities includes prioritizing, managing and following through on multiple tasks, and communicating effectively, courteously and professionally with clients and staff members.

DUTIES

  • Serve as an advocate and experienced eldercare professional on the Elder Care Navigation Team.
  • Provide initial client care assessments to identify parameters of care and capacity. Interface with the family and clients by making visits and communicating frequently. Proved phone and email support to family and clients, answering questions as they arise.
  • Educate family on various care options. Counsel family on care issues and concerns. Make recommendations regarding resources and next steps.
  • Assist families by gathering/sorting important documents necessary for Medicaid and other planning purposes.
  • Act as a “client representative” within the firm, and as a “law firm representative” to the client and family. Liaise with team to find solutions and answers to questions as needed.
  • Research community resources to ensure all firm information of facilities, resources, and materials are up to date to assist clients.
  • Attend meetings for clients at nursing/assisted living facilities, as needed. Document all notes regarding the meeting and decisions properly to the client’s file.
  • Complete regular facility visits to ensure quality of care for our clients.
  • Give presentations to the public various times per year regarding elder care issues.
  • Perform general operational tasks, including but not limited to, answering the phone, copying/scanning, mailings, assisting drop-in clients, filing paperwork, etc., all according to Firm policies and procedures.
  • Work closely with marketing staff to provide guidance and assistance with the promotion of the law firm and its affiliated services throughout the service area, including but not limited to, presenting seminars, writing articles, speaking at events, and relationship building with existing and potential referral sources.
  • Represent the law firm to facilities and the health care community by visiting regularly, creating positive relationships with management and staff.
  • Participate in special events including seminars and marketing events, as requested.
  • Work to make client satisfaction a top priority, finding quick resolution to client complaints and concerns.

QUALIFICATIONS

Bachelor’s Degree in health care, related social work or nursing with an emphasis in geriatrics preferred. Prior experience (3+ years) in the care management, hospital or nursing home setting or related field is preferred. Strong knowledge of long-term care systems (ALF, SNF, home health). Excellent communication and problem solving skills.

  • Knowledge of and efficient computer skills: Windows 8, Internet email, Microsoft Office- Word, Outlook, Excel, Power Point, Publisher, and Time Matters
  • Ability to translate the “legal speak” and “long term care speak” to client/family
  • Professional demeanor with excellent client service skills
  • Excellent verbal and written communication skills (grammar, punctuation, spelling and proofreading skills)
  • High degree of accuracy with strong attention to detail
  • Ability to take initiative and follow tasks through to completion
  • Able to proactively identify solutions to problems encountered
  • Time management skills with ability to meet deadlines and deliver timely work

Job Type: Full-time

Pay: $27.00 - $33.00 per hour
