Job Overview
The RISE Initiative Empowerment Coach plays a critical role in supporting youth participating in The RISE Initiative through trauma-informed counseling, case management, and cohort-based programming.
Working closely with The Rise Initiative's Lead Social Worker, juvenile probation, schools, families, and community partners, the RISE Initiative Empowerment Coach will provide individualized support designed to address emotional, behavioral, and environmental barriers that may impact a participant’s ability to succeed. This role includes home-based visits, school-based visits, office-based counseling, family engagement, and coordination of wraparound services.
In addition to individual case management responsibilities, this position will lead and facilitate structured cohort sessions focused on leadership development, communication skills, decision-making, accountability, and goal setting. The RISE Initiative Empowerment Coach will also coordinate with local partners, organizations, and subject-matter experts to enhance programming and connect participants to meaningful resources and opportunities.
This role requires a relationship-driven professional who can effectively engage youth, manage group dynamics, collaborate across systems, and create a supportive environment that empowers participants toward positive outcomes.
Responsibilities
- Provide trauma-informed counseling and case management services to The RISE Initiative participants.
- Maintain a reduced caseload of no more than 8 Level III participants at a time.
- Provide individual counseling services a minimum of twice weekly per participant.
- Conduct assessments and develop individualized service plans (ISPs) for participants.
- Provide crisis intervention and appropriate referrals as needed.
- Conduct home-based visits, school-based visits, office-based meetings, and family engagement activities
- Lead and facilitate Level II cohort sessions focused on leadership development, communication, decision-making, and goal setting.
- Coordinate and collaborate with juvenile probation, schools, families, and community partners to ensure continuity of support.
- Engage community organizations and guest facilitators to support cohort programming and participant development.
- Assist in refining program curriculum, assessment tools, and service delivery practices.
- Maintain accurate and timely documentation of participant interactions, assessments, referrals, and outcomes.
- Promote a strengths-based, trauma-informed approach in all participant interactions.
- Participate in team meetings, program planning, and continuous improvement efforts.
Requirements
- Master’s degree in Social Work or related field required.
- License eligible required; independent clinical licensure (LICSW preferred).
- Minimum of 2–4 years of experience working with youth, families, or community-based programs preferred.
- Strong understanding of trauma-informed care, youth development, and case management practices.
- Demonstrated ability to build relationships with youth, families, schools, and community partners.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently while collaborating within a multidisciplinary team.
- Valid driver’s license and reliable transportation required.
- Pre-Employment Drug Screen, DHR Child Abuse and Neglect Screening, and Background Check Required.
Pay: $45,000.00 - $53,000.00 per year
