General Definition and Scope of Job
The EMR Triage RN is responsible for the clinical oversight, prioritization, and management of electronic patient communications and care-related workflows within the electronic medical record (EMR). This role ensures timely, accurate, and protocol-driven triage and coordination of patient care needs in an ambulatory setting.
Core Responsibilities
· Monitor and manage EMR in-basket/bucket messages including patient portal messages, refill requests, lab results, and internal staff communications.
· Perform clinical triage of patient-reported symptoms received through the EMR or patient portal and determine appropriate next steps based on established protocols.
· Route messages, clinical questions, and results to the appropriate provider or care team member.
· Provide patient education and guidance through the patient portal or telephone consistent with clinical protocols and provider direction.
· Review laboratory and diagnostic results and ensure timely provider review and patient notification according to organizational policy.
· Prescription and bucket management
· Project Management (examples care coordination, anticoagulation, and colposcopy)
· Prior authorizations for medications and imaging
· Process medication refill requests in accordance with standing orders and provider protocols.
· Identify urgent or emergent patient concerns and escalate appropriately.
· Document all triage assessments, patient communications, and interventions accurately in the EMR.
· Support providers by managing routine electronic communications to improve clinical workflow efficiency.
· Collaborate with medical assistants, providers, and care teams to ensure continuity of care and timely patient follow-up.
Minimum Skills, Experience and Educational Requirements
· Graduate of an accredited school of nursing.
· Has knowledge of nursing and medical principles including disease processes, certain examinations, observations, and laboratory studies; appropriately applies those principles to practice and procedure.
· Possesses and displays the ability to act independently within the scope of the Nurse Practice Act and consistently utilizes good judgment in patient care and the resolution of problems both clinical and non-clinical.
· Consistently utilizes good judgment in the resolution of problems, clinical and non-clinical.
· Maintains professional composure and confidence during stressful situations.
· Demonstrates self-direction and motivation in activities which enhance patient care. Anticipates new problems and is part of the problem-solving process

Required Availability
May be required to work a rotating schedule as the department requires. This includes evenings, holidays and weekends.
Pay: From $35.37 per hour
