Must report to work as scheduled on a regular and reliable basis.
Successfully complete all required pre-service orientation and training.
Ensure confidentiality of all client information and act in accordance with the HIPPA regulations, federal/state laws, and ENOA’s policy and procedures.
Ability to understand and comply with various laws, rules, regulations, policies and guidelines as they pertain to ENOA and ENHSA, Aging Services and the Older American’s Act.
Ability to communicate clearly, both orally and in writing, while establishing effective working relationships with many different people, including peers, coordinators, support staff, providers, community representatives, and the general public.
Be a strong team player with a positive attitude toward working with staff, referral sources, providers, clients and their formal and informal support systems.
Must have good computer skills with general knowledge of Microsoft office and have the ability to learn other computer systems.
Ability to prepare program budgets and forecast expenditures, ensuring that adequate funding is allocated to provide for annual expenditures and operating costs.
Must be well versed, and ensure that staff is as well, of program guidelines, policies, and procedures for Case Management programs.
Supervise program staff and evaluate staffing needs, developing plans of action to meet identified needs.
Direct and oversee employee performance, determine priorities, objectives, and work assignments. Ensure that staff evaluations are conducted according to ENHSA policy.
Identify training needs and develop plans to address those needs.
Hire new staff and ensure all required paperwork is completed and background checks meet agency policy.
Ability to efficiently communicate between ENOA administration and State Unit on Aging administrators and program specialists.
Advocate for the elderly with pertinent agencies/individuals such that the client is able to remain as much independence as the situation permits.
Ensure that needed resources and providers are available or a plan is established to address needs.
Direct and administer in-home services, being responsible for the operation of the Homemaker, Chore, Durable Medical Equipment, Bath Aide, and Personal Emergency Response System services.
Oversee the maintenance of a strong and efficient self-directed care provider list, including recruitment of providers, one-time only background checks and regular maintenance of the provider list.
Ensure agreements and contracts are maintained and kept up-to-date.
Develop and maintain a formal reporting system to maintain program accountability to superiors, government entities and monitoring agencies as warranted.
Assist in developing an ongoing quality assurance process that is equitable for all staff, providers and services within the department.
Oversee assignments to Case Managers, Program Coordinators and Resource Development staff.
Conduct regular staff meetings, ensuring appropriate information is shared in a timely manner and facilitate communication and cohesiveness of the Division.
Maintain staff personnel files in a secured location.
Attend all required meetings.
OTHER JOB DUTIES:
Other job-related duties as assigned by the Executive Director or Executive Director’s designee.
EDUCATION, TRAINING AND EXPERIENCE:
A minimum of State of Nebraska Licensed Registered Nurse, Baccalaureate degree in the field of social work, sociology, gerontology, or applicable human services, with master’s degree preferred. In addition, a minimum of five (5) years’ experience in long-term care, gerontology or community health.
A minimum of three (3) years’ experience in each of the following: development of comprehensive assessments and care plans for the geriatric populations; working with home and community based services and/or case management for the elderly in Nebraska; and direct staff supervision.
Previous experience with the techniques and methods used in the provision of information and referral, counseling, case management, crisis intervention and protective services preferred.
SKILLS AND ABILITIES:
Possess knowledge on computer system including PeerPlace, Word, and Excel.
Excellent writing skills with the ability to avoid subjective commentary and use written skills in report writing and correspondence.
High standard of integrity with good judgement ability and knowledge of professional ethics.
Organized, detail oriented, flexible, and decisive with a strong ability to multi-task, prioritize and use critical thinking skills.
Ability to review and update Case Management policies and procedures.
Ability to work independently.
Ability to lead the Division to meet goals and objectives, as well as, those of the agency.
Ability to effectively and professionally handle work relationships involving employees, management, and the general public.
