We are seeking a dedicated Entry Level Case Manager to support families through Coordinated Family Support services in Alameda County. This hybrid role includes remote documentation and in person client visits for assessments, service coordination, and required signatures.
This position is ideal for someone pursuing a career in case management, social services, or regional center services who has a strong interest in advocacy and systems navigation.
Core Responsibilities:
Coordinate and stabilize regional center services
Support families with public benefits and generic resource applications
Assist with medical appointment coordination and follow through
Provide advocacy and systems navigation support
Maintain accurate, timely, and compliance aligned documentation
Conduct in home or community based visits as needed
Required Qualifications for CFS:
Strong understanding of person centered practices
Ability to coordinate multiple services and providers effectively
Experience navigating public benefits such as SSI, Medi Cal, IHSS, or similar programs preferred
Excellent documentation and form completion skills
Strong advocacy skills with professional communication
Ability to manage time independently in a hybrid environment
Reliable transportation for travel within Alameda County
Ability to work legally in the United States
Preferred:
Background in social work, psychology, human services, or related field
Experience supporting individuals with developmental disabilities
Familiarity with regional center systems and Title 17 services
We are looking for someone organized, compassionate, and confident in navigating complex systems while maintaining professionalism and accountability.
Job Type: Part-time
Pay: $25.00 - $35.00 per hour
Expected hours: 20 per week
