Entry-Level Case Manager, Housing Reentry Program

Location: Oakland, California
Position Type: Full-time, 40 hours per week

Compensation: $24-$28 per hour, depending on experience
Benefits: $600/month stipend to help offset health insurance or other eligible benefit costs
Status: At-will employment; first 90 days are an introductory period

Position Summary
A faith-based community organization in Oakland is seeking a compassionate, dependable, and highly organized Entry-Level Case Manager to support participants in a housing reentry program. This position provides basic case management services for individuals returning to the community and working toward housing stability, personal growth, and successful reintegration. The ideal candidate is people-centered, professional, resourceful, and committed to serving individuals with dignity, consistency, and accountability.
Key Responsibilities

  • Provide basic case management services to participants enrolled in the housing reentry program.
  • Conduct intake, needs assessments, service planning, and goal tracking.
  • Support participants with referrals to housing, employment, healthcare, behavioral health, substance use recovery, transportation, and other community resources.
  • Maintain regular contact with participants through office meetings, phone calls, check-ins, and field-based follow-up as needed.
  • Encourage participant progress, problem-solving, accountability, and engagement with program goals.
  • Document case notes, referrals, service plans, outcomes, and participant updates accurately and on time.
  • Enter and maintain participant data in Salesforce and other required systems.
  • Coordinate with program staff, community partners, service providers, and referral agencies.
  • Maintain confidentiality and handle sensitive information with professionalism and sound judgment.
  • Support a respectful, trauma-informed, and mission-driven environment.
  • Work flexible hours as needed to meet program and participant needs.
  • Required Qualifications
  • High school diploma or equivalent required; associate’s or bachelor’s degree in social work, human services, psychology, criminal justice, or a related field preferred.
  • Entry-level candidates are encouraged to apply; experience in case management, reentry, housing services, homelessness services, social services, or community-based support is preferred.
  • Superb computer skills are required.
  • Experience with Salesforce is required or strongly preferred.
  • Strong working knowledge of Microsoft Word and Microsoft Office is required.
  • Must be experienced with basic computer systems, email, data entry, online forms, and web-based platforms.
  • Familiarity with AI tools is a plus.
  • Must be able to think critically, solve problems, and exercise sound judgment.
  • Must be comfortable working flexible hours, including occasional evenings or schedule adjustments.
  • Strong written communication, verbal communication, and organizational skills are required.
  • Ability to work effectively with diverse populations, including individuals impacted by incarceration, housing instability, trauma, or poverty.

Introductory Period and Employment Status
The first 90 days of employment are considered an introductory period for training, evaluation, and overall fit. Employment is at-will, which means either the employee or the organization may end the employment relationship at any time, with or without cause or notice, subject to applicable law.
