We are hiring 3 Pharmaceutical Sales Representatives on a 1099 basis to build our referral network across Washtenaw County. The comp structure pays you for every patient you bring in and every partnership you sign, and the draw activates when you produce. We deliver bioidentical hormone replacement therapy (BHRT), testosterone replacement therapy (TRT), peptide therapy, sexual wellness (P-Shot, O-Shot, shockwave therapy), medical aesthetics, plasma soft surgery (PlexrPlus, Plexrplasty, CAT Resurfacing), IV nutrient therapy, hair restoration, and physician-supervised medical weight loss. Our patients are accomplished professionals, performance-driven adults, perimenopausal and postmenopausal women, and high-net-worth retirees who take their long-term health seriously. They expect clinical excellence and a concierge experience. We deliver both.

Our clinical team is led by Gandhi Bhattarai, FNP-BC, PMHNP-BC, a board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner. Every treatment at Arbour is supervised by licensed clinical providers. This is not a medspa. This is regenerative medicine.

The rep who builds a partnership network in this territory is building a long-term annuity. The Role Field-based B2B referral development. You build relationships with physicians, specialty practices, pharmacies, and other clinical decision-makers in the Ann Arbor metro. Every relationship you create is a long-term recurring revenue channel for you.

You will be representing real medicine. BHRT. TRT. Peptide therapy. PRP. Shockwave therapy. Plasma soft surgery. The kind of services patients pay cash for because nothing else worked.

What You Will Be Representing

VIP / Platinum Membership: $799 per month

Gold Membership: $399 per month

BHRT Membership: $299 per month

Medical Weight Loss Program: $299 per month

Peptide Therapy Membership: $299 per month 

Vampire Facial (PRP Microneedling): $799 per session

Plexrplasty Full Face Lift: $5,000 and up

CAT Resurfacing Series: $1,999 and up

Every paid first-time service of $250 or more triggers your $100 commission. Every qualifying membership pays you $50 per month for 6 months.

Compensation: The Real Numbers How you earn $50 per referred patient, per month, for the first 6 months that patient maintains an active, paid qualifying membership. That is up to $300 per membership referral over time. Qualifying memberships: BHRT, Medical Weight Loss, Peptide Therapy, Gold, and VIP / Platinum. $100 per qualifying first-time treatment.

Paid the first time a referred new patient books, attends, and pays in full for a qualifying treatment ($150+ list price).

No payment on no-shows or unpaid visits. Partner-business override. Any partner business you sign (gym, physician group, pharmacy, wellness brand) is enrolled at $25 per referred patient per month for 6 months plus $50 per qualifying first-time treatment. You keep your full rep commission on top. No cap on earnings.

Year 1 income example (conservative, realistic ramp)

Months 1 to 2: Onboarding and ramp. No commissions yet. You are building your pipeline and signing your first partnerships.

Month 3: Commission gate cleared (1 partnership signed OR 5 patient referrals delivered). You start earning. Sample month: 3 patient memberships + 4 first-time treatments = $150/mo recurring + $400 oneoff. Total month: $550.

Month 6: Draw activated (5 partnerships signed OR 10 patient referrals delivered). $1,000/mo draw kicks in. Sample month: 15 active memberships referred + 6 first-time treatments. Recurring $750 + one-off $600 + draw $1,000. Total month: $2,350.

Month 12: Steady state. 30+ active memberships referred + 8 first-time treatments per month. Recurring $1,500 + one-off $800 + draw $1,000. Total month: $3,300. Annualized run-rate: $40,000.

Top producers (15+ qualified referrals per month at steady state): $6,000 to $10,000 per month. Annualized: $72,000 to $120,000+. Payment terms (this is how we protect every rep and run a clean program)

Commissions are earned only after the patient has booked, attended, and paid in full for the qualifying treatment.

Qualifying treatment = paid service of $150 or more. Consultations and skin assessments alone do not qualify.

Membership commissions paid monthly, in arrears, only while the patient holds an active paid qualifying membership. Refunds, chargebacks, adjustments, no-shows, and cancellations reverse the associated commission. Referrals must be net-new patients.

Attribution is set at booking. Patient identifies the referring rep or partner on the intake form. Disputes resolved by clinic leadership.

Commissions paid on the 15th of each month, via direct deposit. 1099-NEC is issued annually.

This is field-based B2B partnership development and consultative network referral. Not door-to-door sales. We do not knock on residential doors. Not retail. You are not standing in a store. Not a 'finder's fee' one-time bounty. This is an ongoing 6-month recurring commission per membership plus per-treatment commission. Designed to compound. Not a salaried W-2 role. This is a 1099 independent contractor role with a performance-activated draw. If you want pure salary with no upside, this is not for you.

Qualifications :

• 2+ years of medical device, surgical, pharmaceutical, or healthcare sales experience.
• You should be a top 25 percent performer at your most recent sales role. We hire up.
•  Existing relationships with physicians, gym owners, or wellness business operators in the Ann Arbor metro strongly preferred.
• Comfort discussing medical aesthetic, hormone, peptide, and sexual wellness services in a clinical, professional manner.
• Bachelor's degree preferred. Strong territory experience will be considered in lieu of degree.
• Self-directed, organized, and accountable to a pipeline you own.  Reliable transportation and a clean driving record.
• Days 1 to 14: Onboard fast Sign contractor agreement, W-9, and direct deposit setup.
• Receive your Arbour brand bible, sales materials, and CRM access.
• Build your written 90-day territory plan. Submit to clinic leadership.
• Days 15 to 45: Pipeline build 15 qualified outreach contacts per week minimum. 5 in-person partner meetings per week minimum.
• Goal: Sign your first partnership by day 30. Goal: Hit the commission qualification gate (1 partnership OR 5 referrals) by day 45.
• Days 46 to 90: First referrals and draw activation Goal: 3+ qualified patient referrals per month by day 60.
• Goal: 5 partnerships signed OR 10 qualified referrals delivered by day 90. This unlocks your $1,000 per month draw.
• Day 90 review with clinic leadership: pipeline health, results, support needed, territory expansion.

Common Objections, Answered Is this MLM? No. There is no team to build and no recruiting. You earn for patients you personally refer who become Arbour patients. That is it.

How do I know I'll get credit for a referral? Every patient identifies the referring rep or partner on the intake form at the time of booking. Attribution is set at booking and tracked in our CRM. You will see your pipeline in real time.

What happens if I do not hit the commission gate? You earn nothing until you have signed 1 partnership or delivered 5 referrals. We do not pay for activity, we pay for results. If you cannot produce, this is not the right role.

Will I have to cold-call physicians? Yes. This role requires you to identify, contact, and meet with potential referral sources. We provide the training, the materials, and the in-clinic experience. You provide the legs.

FAQ How are commissions tracked and paid? Every referral is logged in our CRM at the time of patient booking. Commissions are calculated automatically and paid on the 15th of each month for the prior month's qualifying activity, via direct deposit.

What CRM and sales tools do you provide? You receive access to our partnership CRM, the Arbour brand bible, a complete sales deck, lunch-and-learn materials, branded literature, and a mileage-tracked expense template.

What if I leave or am terminated? Earned commissions for completed patient visits are paid through your last full month. Future month membership commissions on patients you previously referred terminate at separation. This is standard for 1099 referral programs.

Who handles inbound leads from my partners? Arbour's patient coordinator team handles all inbound. You are expected to schedule patients yourself. Your job is the partnership and the referral. W

How are taxes handled? You are a 1099 independent contractor. You receive a 1099-NEC annually. You are responsible for your own taxes, mileage tracking, and any business expenses. We strongly recommend you consult a tax professional.

What happens if two reps claim the same patient? Attribution is set by the patient at booking. The patient names the referring person or business on their intake form. That decision is final. Disputes are resolved by clinic leadership based on intake records.
