Doors of Hope is a nonprofit organization, providing unique outpatient substance use treatment for women and a trauma-informed 12-month reentry and recovery program for women returning to the community following incarceration. This is a part time, 20 hour/week role, with Fridays off. The Evening IOP Case Manager provides essential support services to women enrolled in the Intensive Outpatient Program during evening hours. This role works closely with the Evening IOP Counselor to ensure smooth program flow, consistent client engagement, and comprehensive case management support.
The Evening IOP Case Manager assists with client check-ins, continuing care planning, and coordination of community-based services to support sustained recovery and successful re-entry.

Essential Duties and Responsibilities
Under the supervision of the Clinical Program Director and in collaboration with the IOP treatment team, the Evening IOP Case Manager will:
- Support evening IOP operations, including client check-ins and attendance tracking
- Provide ongoing case management services to IOP clients throughout their treatment episode
- Collaborate with clients to develop and update continuing care and discharge plans
- Coordinate and facilitate referrals to community partners and external providers (e.g., mental health, medical, housing, employment, legal, and recovery resources)
- Maintain clear, timely, and accurate case management documentation in the Electronic Health Record (EHR)
- Communicate regularly with the Evening IOP Counselor to support coordinated care and treatment planning
- Assist clients in identifying barriers to treatment participation and accessing appropriate supports
- Advocate for clients while encouraging accountability and self-sufficiency
- Contribute to a trauma-informed, respectful, and recovery-oriented program environment
QUALIFICATIONS
Education and Experience
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field required (Master’s degree preferred)
- Experience in case management, substance use treatment, behavioral health, or re-entry services preferred
- Knowledge of community resources and referral processes strongly preferred

WORKING CONDITIONS

- Schedule: Monday through Thursday, 4:30 PM – 9:30 PM
