Position Overview:
The Expanded School Mental Health (ESMH) Case Manager provides comprehensive case management services to students experiencing social, emotional, and behavioral challenges. This role works collaboratively with school staff, families, and community providers to support student well-being, improve access to mental health services, and promote academic success.
Key Responsibilities:
- Conduct assessments to identify student needs, strengths, and service gaps
- Develop, implement, and monitor individualized service plans
- Provide ongoing case management, including coordination of school-based and community mental health services
- Collaborate with teachers, counselors, administrators, and external providers
- Facilitate referrals to appropriate behavioral health, medical, and social services
- Maintain accurate and timely documentation in compliance with agency and regulatory standards
- Support crisis intervention and safety planning as needed
- Engage families to promote student success and continuity of care
- Participate in multidisciplinary team meetings and professional development opportunities
Qualifications:
- Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related field
- Experience working with children and adolescents in school or community settings
- Knowledge of trauma-informed care, behavioral health systems, and case management practices
- Strong communication, organizational, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
Preferred Qualifications:
- Experience in school-based mental health programs
- Familiarity with special education processes and IEP/504 plans
- Bilingual skills are a plus
Pay: From $30.00 per hour
