Job Summary
We are seeking passionate and dedicated Experience Caregivers ages 43 - 55 years of age to join our team and provide exceptional support to individuals with diverse needs. In this vital role, you will deliver personalized care that promotes independence, safety, and well-being. Whether working in a home setting, assisted living facility, or group home, your compassionate approach will make a meaningful difference in the lives of those you serve. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by checking vital signs and observing for any changes in condition
• Reminder of medications accurately following care plans and HIPAA regulations to ensure client safety
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
• Maintain cleanliness and hygiene in clients’ living spaces through regular cleaning and sanitation
• Support clients during outings or community activities, promoting social engagement and independence
• Document care activities, updates on patient status, and any incidents clearly and promptly in accordance with care protocols

Skills

• Proven experience in home health, assisted living, nursing homes, or group homes with a focus on senior or disability care
• Knowledge of patient observation techniques and vital signs monitoring
• Strong ability to assist with ADLs and provide compassionate patient care for individuals with dementia, Alzheimer’s, autism, or developmental disabilities
• Experience with specialized care such as memory care or hospice support is highly valued
• Ability to perform heavy lifting safely using equipment like Hoyer lifts when required
• Excellent communication skills to collaborate effectively with team members and family members
