Join our dedicated team as an Experience Mental Health (HCS) Home Community Service provider, where you will play a vital role in delivering compassionate, personalized mental health support directly within clients’ homes and community settings. Responsibilities

• Provide direct mental health services and support within clients’ homes, ensuring a safe and nurturing environment.
• Conduct assessments to identify individual needs, develop personalized care plans, and implement therapeutic strategies aligned with best practices.
• Manage crisis situations effectively through crisis intervention techniques and crisis management protocols to ensure client safety.
• Support clients with behavioral management strategies, including applied behavior analysis (ABA), to improve daily functioning and social skills.
• Administer medications accurately following HIPAA guidelines and medical documentation standards, maintaining detailed medical records.
• Collect and analyze data related to client progress, behavior patterns, and treatment outcomes to inform ongoing care adjustments.
• Collaborate with families, caregivers, social workers, and healthcare professionals to coordinate services and ensure comprehensive support for each individual.

Skills

• Experience working with children and adolescents facing mental health or developmental challenges.
• Proficiency in crisis intervention, crisis management, and de-escalation techniques to handle emergency situations confidently.
• Knowledge of working with individuals with autism spectrum disorder (ASD), disabilities, or developmental delays; familiarity with special education principles is a plus.
• Ability to perform vital signs measurement and monitor physical health indicators as part of holistic patient care.
• Strong understanding of social work principles, patient care standards, medical terminology, and medical records management.
• Skilled in intake processes, behavioral health assessments, data collection, and documentation compliance under HIPAA regulations.
• Competence in medication administration procedures while adhering to safety protocols and legal requirements.
