Duties
- Conduct thorough patient assessments to identify social, emotional, and environmental needs, including mental health counseling and behavioral therapy.
- Develop personalized care plans that incorporate case management, crisis intervention, and clinical counseling strategies.
- Facilitate individual and group therapy sessions utilizing motivational interviewing, cognitive behavioral therapy (CBT), and applied behavior analysis (ABA) techniques.
- Coordinate with interdisciplinary teams to ensure seamless delivery of services such as hospice care, Alzheimer's and dementia care, PTSD support, and working with individuals with developmental disabilities.
- Manage medical documentation accurately within Electronic Medical Record (EMR) systems while adhering to HIPAA regulations and maintaining confidentiality of medical records.
- Assist with intake procedures, discharge planning, and post-acute care planning to support patient transitions across settings like assisted living or outpatient clinics.
- Engage in data collection for clinical research projects related to behavioral health, mental health counseling, or special education initiatives.
Qualifications
- Proven experience as a social worker in skilled nursing or inpatient settings with familiarity in hospital experience and nursing home environments.
- Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards.
- Expertise in child & family counseling, behavioral health therapies—including behavioral therapy, crisis management, and working with individuals with autism or disabilities.
- Skills in diagnostic evaluation, mental health counseling, case management, and outpatient clinical settings.
- Ability to perform patient assessments effectively while demonstrating excellent communication skills for intake and group therapy sessions.
- Familiarity with EMR systems and healthcare regulations such as HIPAA; experience working with ICD coding is preferred.
- Certifications in social work (e.g., LCSW or equivalent), along with training in hospice care, addiction counseling, or special education is advantageous.
- Passion for working with diverse populations including geriatrics, pediatrics, and individuals requiring dementia or PTSD care.
