<Duties>

  • Perform thorough home assessments for patients across various settings, including post-acute, outpatient, and clinic environments.
  • Evaluate vital signs, physiological status, and overall health conditions to determine appropriate care interventions.
  • Review medical records and diagnostic evaluations to inform patient care planning and discharge processes.
  • Collaborate with interdisciplinary teams to develop and update individualized care plans that meet patient needs.
  • Conduct case management activities, including discharge planning and utilization management, ensuring seamless transitions of care.
  • Document all assessments accurately while maintaining strict adherence to HIPAA regulations and medical terminology standards.
  • Communicate effectively with patients, families, and healthcare providers to coordinate services and optimize patient outcomes.

<Skills>

  • Proven critical care experience with a strong background in hospital medicine, emergency medicine, or ICU settings.
  • Experience working within Level I Trauma Centers or Level II Trauma Centers is highly desirable.
  • Knowledge of infectious disease care, post-acute care, and outpatient clinic operations enhances your ability to deliver comprehensive assessments.
  • Familiarity with medical-surgical nursing, geriatrics, behavioral health, pediatrics (PICU), and inpatient/outpatient environments.
  • Strong understanding of physiology, vital signs monitoring, diagnostic evaluation, discharge planning, and case management processes.
  • Proficiency in medical terminology, medical records documentation, utilization management, and intake procedures.
  • Ability to adapt quickly across diverse clinical settings such as emergency rooms, OR environments, managed care organizations, and hospital-based units.
