The Child and Family Team Meeting Facilitation program is a partnership between Fred Finch, San Diego County Child Welfare Services, Behavioral Health Services, and Probation. Child and Family Team promotes the overall well-being of youth up to 21 years old through enhanced communication and collaboration with the people most important in their lives, including family and caregivers.
When a specific need surfaces, Child and Family Team staff take the lead in organizing, scheduling, and facilitating a meeting. The Facilitator starts by working with the youth and family or caregivers to discuss who specifically needs to participate in the treatment plan, which may include Protective Social Workers, Protective Social Worker Supervisors, Probation Officers, teachers, psychiatrists and therapists. The facilitator also convenes natural support important to the family and youth such as neighbors, relatives, family friends, religious leaders and sports coaches.
What You Will Do:
The Facilitator is responsible for Child and Family Team Meeting Facilitation to include all aspects of meeting scheduling (location, venue, and time) with assigned families in accordance with published Child and Family Team standards and principles. Under the direction of the Lead CFT Facilitator, the Facilitator develops an individualized plan to meet the goals of the youth and family. The plan should be designed to capitalize on the strengths of assigned families while ensuring coordination of both professionally based and community-based supports. The Facilitator is responsible for insuring that there is only one CFT Team process per family to improve family engagement, and to improve transition to a permanent, loving home at the earliest possible time.
