POSITION DESCRIPTION:

The Family Advocate works under the direct supervision of the Community Services Division Director. This position is responsible for the full case management and social service activities for clients.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

  • Position will serve clients throughout the five county service area and will be required to travel from site to site in order to meet the needs of the community members.
  • Provide case management services to individuals and families including direct services to clients by assisting them in determining strengths, needs, and goals.
  • Responsible for the recruitment and enrollment of eligible individuals/families in the program through outreach activities.
  • Models appropriate advocacy skills for clients in working with community agencies and professionals.
  • Link clients with appropriate community resources as requires/needed.
  • Responsible for maintaining forms/case files relating to each client/family ensuring confidentiality as required by law and Coastal Georgia Area CAA, Inc. policy.
  • Responsible for monitoring, inputting, and tracking all client information by use of the EasyTrak database on an on-going basis.
  • Attend community meetings when appropriate and beneficial to the program.
  • Responsible for creating and maintaining an individualized service plan in coordination with a formal assessment and appropriate intake assessment/eligibility for clients.
  • Participates in the development and strategy of beneficial education for clients served and the community-at-large.
  • Establish and maintain relationships with other agencies and organizations in the community to meet community needs and to increase program success.
  • Interview and assist clients with completion of application forms which include: SNAP, SSI, SSD, WIC, Medicaid/Medicare, employment, housing assistance, education, healthcare, childcare, and veterans’ services.
  • Responsible for appropriate follow-up and discharge planning on a 30-60-90-day period.
  • Prepares regular and special reports, prepares and maintains appropriate and proper documentation.  Conducts interim and special reviews and home visits to assist with client gaining self-sufficiency.
  • Responsible for ensuring all safety rules and regulations are followed according to the Authority’s policies and procedures, as well as state and federal regulations.
  • Follow the Authority’s Procurement and Personnel Policies and Procedures.

OTHER REPONSIBILITIES:

  • Participates in Community activities related to the goals and objectives of the Authority and activities sponsored by the Authority.
  • Provides crisis intervention as needed and appropriate.
  • Provides education to families relating to topics such as budgeting, safety, health, and other family issues as necessary.
  • Attending meetings, trainings, and individual case staffing.
  • Perform other duties as are assigned.

SUPERVISORY LEVEL:

No direct supervision is performed.

EMPLOYLMENT STANDARDS OR JOB REQUIREMENTS:

Education, Training, Special Licenses: Completion of a Bachelor degree in Human Sciences, Sociology, Psychology, Social Work or related field or, completion of an Associate in related fields with at least 2 years’ experience. Must possess a valid Georgia Driver’s License, along with a clean MVR of at least 3 years. Must have an acceptable Criminal Background Check and pre-employment Drug Screen.

Experience: Preferably at least two (2) years’ experience in Social Services and/or human services. Experience must include use of spreadsheet software such as Microsoft Excel; database software; typing skills and computer literacy.

Skill at: Requires strong interpersonal skills; customer service skill in dealing with the general public; communicating with a diverse population; and business telephone procedures and etiquette. Must be able to communicate written and verbally in the structure and content of the English language with attention to spelling, composition and grammar. In addition, must have the ability to organize office files, equipment, etc. and prioritize daily work.

Equipment to be used: Must possess valid driver’s license. Must be knowledgeable in the use of computers, fax, copiers, business telephones, and 10 key calculators.

TYPICAL PHYSICAL DEMANDS:

Must be able to lift 30 to 50 pounds and carry 20 to 30 pounds. The job also requires manual dexterity sufficient to operate standard office equipment and requires normal range of vision and hearing or eyehand coordination. In addition, the job requires sitting at a desk for extended periods of time. May require driving to and from surrounding counties.

Must submit to drug screening if drug use is suspected.

TYPICAL MENTAL DEMANDS:

Must be able to interpret and analyze statistical and financial information and identify problems; prepare technical reports for use by management; develop, monitor and submit budget reports; making oral presentations before groups; and be able to communicate written and orally using generally accepted grammar
