Todos Juntos Learning Center is seeking to hire a Family Case Manager to help support our Saludables Juntos Program to maintain the depth of services we offer our families. We are looking for someone who thrives in a fast-paced (and fun!) environment, learns quickly, thinks creatively, communicates well, and excels at helping people creatively and compassionately solve problems.

Job Title:

Family Case Manager

Reports to: Saludables Juntos Program Manager

Primary Purpose/Function of Position:

The Todos Juntos Family Case Manager will support the Saludables Juntos Program at Todos Juntos, which houses our wraparound and health-centric initiatives. They will work closely with our Programs team and early childhood education (ECE) students and families to help ensure each family has access to the resources they need to thrive in learning and in life. This person is passionate about taking strengths-based approaches to help families get to where they want to be.

The Family Case Manager is responsible for:

  • Case management (about 75% of time) for identified ECE students needing Early Childhood Intervention referrals.
  • Communication with parents of ECE students regarding their child’s case, any subsequent referrals, and explanation of situations of abuse and neglect that parents need to understand.
  • Coordination and facilitation of health-related activities and events for students as needed.
  • Provide support services such as making referrals, developing individual service plans using assessment results, and crisis intervention with the goal of identifying service needs and implementing plans to meet those needs.
  • Complete Family Needs Assessments to locate, match, and manage external resources & services to meet students’ needs.
  • Build relationships within the Central Texas community and maintain positive partnerships with external organizations.
  • Opportunity to supervise Social Work intern(s) from local universities.
  • Complete timely documentation, evaluations, and data reporting as required.
  • Participate in Early Childhood Coalition meetings, trainings, and advocacy initiatives.
  • Oversee and maintain the organization's resource board, keeping resources current and relevant while identifying and adding new community resources each month.
  • Assist with the creation of flyers and promotional materials in Canva for program, events, and community outreach activities.
  • Other duties as assigned.

Qualifications:

  • Fluency or high proficiency in Spanish required.
  • Bachelor’s degree in social work, human services, or related fields, Social Work degree(s) highly preferred.
  • Knowledge of local, state, and federal laws and regulations that govern the administration of agency programs.
  • Minimum of one year of professional experience working with families, adults, and/or youth in providing either direct service or case management services.
  • Demonstrated cultural competence when engaging with diverse audiences including students and families, Todos Juntos’ staff, volunteers, and community partners.
  • Comfortable with routinely shifting demands.
  • Ability to work independently while functioning effectively as a member of a larger team.
  • Valid Texas driver’s license & access to reliable transportation.
  • Occasional use of personal vehicle will be asked for seasonal events
  • Must be eligible to work in the United States.
  • Quick learner with the ability to adapt to changing community needs.
  • Has strong communication and interpersonal skills.
  • Is a proactive problem solver.
  • Help support and coordinate our annual Saludables Juntos Health Fair.

Preferred knowledge, skills, and abilities:

  • Experience working with Apricot (Bonterra) CRM software, Canva and Google Suite.
  • Knowledge of Central Texas nonprofit sector and government agencies–specifically community resources including healthcare, mental healthcare and basic needs for children.
  • Experience in the application process of government assistance programs such as SNAP, WIC, Medicaid, CHIP, TANF, etc.

Hours:

Full-time employee, Monday through Friday, 8:00 am to 5:00 pm. Occasional evenings and weekends (one to two events per semester). This role is a majority in person role at our offices with the opportunity to work remotely on occasion and/or as needed after initial probationary period is complete

Compensation:

  • $44,000 annual salary
  • Comprehensive healthcare benefits, including medical, dental, and vision, and 100% premium coverage by Todos Juntos for employees only.
  • Four weeks (Thanksgiving, Winter, Spring Break) holiday pay, for a total of 26 paid holidays per year.
  • 10 days flexible PTO.
  • Free on-site part-time childcare for children 12 months - 4 years with prior notice & arrangements, pending space availability.
  • $25 monthly cell phone stipend
  • Mileage reimbursement for business-related travel ($0.725/mile).
  • Up to $500 annually towards professional development and support, pending a one-year commitment toTodos Juntos.
  • Free onsite parking.
