<li>Assess the level of risk to the child for additional injury or harm, including whether the child is in imminent danger, and may remove the child from the family as the situation warrants, placing a child in a protected environment.</li>
<li>Perform needs assessments to determine treatment options for families and children reported to be abused/neglected.</li>
<li>Testify and prepare pre-dispositional and progress reports, maintain case files, and develop briefing reports for the community child protection team. Develops &ldquo;informal adjustment&rdquo; plans with families and children to divert children from the juvenile justice system.</li>
<li>Develop case plans to assist families and children to become more self-sufficient in a safe and nurturing environment and to ensure a permanent home for the child.</li>

<div><p>The Bryan County Family Connection is now accepting applications for a Full Time <b>Case Manager.</b></p><p>
This position is to establish baseline needs assessments; provide case management and coordination of services; provide intake services; maintain records; and provide a variety of administrative duties to facilitate the effectiveness and efficiency of the program.</p><p><b>
ESSENTIAL FUNCTIONS</b></p><p><b>
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.</b></p><p>
Establish baseline needs assessment for each client and his/her family. Identifies needs for supportive services by observation of behavior and review of records. Listen carefully to concerns and ask effective questions to assist individual in expressing true feelings. Assist families to understand the case plan and program goals. Advise families and of other resources and make appropriate referrals as needed with supervisors&rsquo; approval.</p><p>
Provide a variety of case management and coordination of services. Schedule visits with individuals and/or families as necessary. Assist with attendance to required sessions and appointments as specified in service plan. Establish and maintain effective working relationships with human service delivery agencies as appropriate.</p><p>
Provide intake services as delegated by receiving referrals from Bryan County Family Connection Executive Director and/or Program Coordinator. Collect information and determine validity of referral in accordance with established procedures. Ascertain place of residence and the need for individualized plan. Collect data as required by Bryan County Family Connection Annual Operating Plan.
</p><p>Maintains records on individuals/families enrolled in the program, contacts, case management services, ensure effective follow-up and provide data for required reporting. Maintain accurate up-to-date records of all assessments, plans, interventions, family involvement, outside agency communication, surveillance activities and other contacts. Write concise, accurate and organized progress notes and other case documentation. Prepare required documents, case forms, and other materials in compliance with laws, rules, policies and procedures. Maintains confidentiality of enrolled records in accordance with state law.</p><p>
Provide a variety of administrative duties to facilitate the effectiveness and efficiency of the program. Develop and maintain and in-depth knowledge of departmental policies, state laws, rules and procedures as they relate to the job duties and the supervision program through regular review of documents and attendance at training events. Represent the program and the Family Connection Board in a professional manner by speaking to community groups, other agencies and related organizations about the Case Management Program. Participate actively in supervision and staff development activities by regular attendance at staff meetings and conferences.</p><p>
Attend Bryan County Family Connection monthly collaborative meetings and other meetings pertinent to the position.</p><p>

This is a full-time opportunity, Monday &ndash; Friday with typical hours being from 8:00 AM- 4:30 PM with paid overtime as needed including after-hours and weekend on call rotation. All paid overtime must be pre-approved by leadership.</div><div><br>
The primary purpose for this position is to work with families in which child abuse and/or neglect has occurred. The focus is to decrease the likelihood of continued abuse while preserving families. As a Social Work Case Manager, there will be times in which you will have to remove children. This can be a difficult and challenging process.</div><div><b><br>
Responsibilities include but are not limited to:</b></div><div><ul><li>
Assess the ongoing risk to children, develop safety plans, assess the strengths, and needs of the child and family, and make recommendations to various county attorneys and courts.</li><li>
Frequent interaction with law enforcement, medical/mental health community partners, schools, etc.</li><li>
Requires the ability to apply Iowa Code, agency policy, and sound professional judgment to highly complex situations involving crises and or chronic abuse or neglect</li><li>
All cases assigned to persons in this position require home visits, detailed interviews, and thorough narrative documentation</li><li>
Troubleshoots problematic situations to find innovative solutions to present to administration and the supervisor. Confronts crisis situations and high stress situations in a calm and orderly manner. Maintaining confidential information as required by the agency and assuring safety is a top priority for all in the office</li><li>
Obtain information from a wide variety of sources, including written materials, telephone conversations and through computer databases and to be able to read and interpret the data</li><li>
Identifies rules, principles, or relationships that explain reasonable facts, data, or other information that analyzes information and draws accurate conclusions</li><li>
Interacts in a team environment to manage group processes, facilitates, and encourages cooperation, pride, trust, and group identity, fosters commitment and team spirit</li><li>
Works with others to achieve goals</li><li>
Within an office environment, provides Division support to large segment of agency through managing internal and external requests for information and legislative reviews requiring comprehension of request, independent judgment and adherence to agency formatting of timely response or communication. Interact with employees and customers in a positive customer service manner to resolve problems and answer customer inquiries. Customer contacts may include correspondence, telephone, or walk-ins to the local office</li><li>
This position will be required to work or be on standby during nights and weekends on a rotational basis.</li></ul></div><div><b><br>
