<li>Investigate reported incidents of child abuse, neglect, or dependency, determine whether the incident is substantiated, and develop recommendations to a Juvenile Court or County Director for disposition.</li>
<li>Assess the level of risk to the child for additional injury or harm, including whether the child is in imminent danger, and may remove the child from the family as the situation warrants, placing a child in a protected environment.</li>
<li>Perform needs assessments to determine treatment options for families and children reported to be abused/neglected.</li>
<li>Testify and prepare pre-dispositional and progress reports, maintain case files, and develop briefing reports for the community child protection team. Develops &ldquo;informal adjustment&rdquo; plans with families and children to divert children from the juvenile justice system.</li>
<li>Develop case plans to assist families and children to become more self-sufficient in a safe and nurturing environment and to ensure a permanent home for the child.</li>
<li>Develop discharge/transition programming to assist families and children to attain and maintain self-sufficiency in a safe environment.</li>
<li>Monitor and assess the service capability of treatment programs and providers to ensure consistency with the case plan.</li>

<p></p><div><div>The Iowa Department of Health and Human Services (HHS), Division of Family Well-Being and Protection, Bureau of Child Protective Services, is seeking a Child &amp; Family Services Case Manager to join our team!</div><div><br>
This position is located in Burlington and will cover that and surrounding counties.</div><div><br>
This is a full-time opportunity, Monday &ndash; Friday with typical hours being from 8:00 AM- 4:30 PM with paid overtime as needed including after-hours and weekend on call rotation. All paid overtime must be pre-approved by leadership.</div><div><br>
The primary purpose for this position is to work with families in which child abuse and/or neglect has occurred. The focus is to decrease the likelihood of continued abuse while preserving families. We do our best to keep families together, however, there are times in which this is not possible. In these circumstances, children may have to be removed from parental custody and placed in another home, preferably still with family. As a Social Work Case Manager, there will be times in which you will have to remove children. This can be a difficult and challenging process.</div><div><b><br>
Responsibilities include but are not limited to:</b></div><div><ul><li>
Assess the ongoing risk to children, develop safety plans, assess the strengths, and needs of the child and family, and make recommendations to various county attorneys and courts.</li><li>

The <b>Case Manager</b> will report to the Clinical Supervisor. In collaboration with other members of the clinical team, the case manager is responsible for serving as a liaison for the treatment team, client, families and collateral service providers.</p>

Responsibilities:</b></p>
<ul><li>Maintains assigned caseload and ensures the proper implementation of in-home services in accordance with referrals made by DCYF.</li></ul>
<ul><li>Works collaboratively with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular service specific documentation.</li></ul>
<ul><li>Develops and collaborates on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.</li></ul>
<ul><li>Adheres to all documentation requirements of service delivery including assessment writing and report writing.</li></ul>
<ul><li>Participates in reporting progress to the court system through in person appearances and written reporting as requested.</li></ul>
<ul><li>Provides parent education and supervised visitation to children, adults, and families in home and community-based settings.</li></ul>
<ul><li>Offers parent coaching and support including provision of effective parenting strategies, meeting healthcare and educational needs of children, understanding developmental milestones, meal preparation and budgeting.</li></ul>
<ul><li>Enforces supervised visitation expectations as identified by DCYF to ensure child safety.</li></ul>
<ul><li>Interfaces with DCYF, the NH court system, schools, behavioral health providers, and any other involved members of the treatment team as necessary to provide quality service delivery.</li></ul>
<ul><li>Successfully assesses client risk on an ongoing basis and consult with clinical supervisor regarding any safety concerns.</li></ul>
<ul><li>Transportation of clients, including children and adults, requiring car seat and booster seat installation.</li></ul>
<ul><li><b>On-call rotation</b>: responsible for responding to phone to provide support to families as utilized.</li></ul>
<ul><li><b><i>Early evening availability required for working hours. Most visits to clients are in the afternoon or early evenings. Must have flexible availability to work due to this. Schedule/hours are subject to our clients needs.</i></b></li></ul>
<ul><li>Ability to successfully document all clinical contacts and documentation through Electronic Health System.</li></ul>
<ul><li>Other duties as assigned.</li></ul><br>

Qualifications:</b></p>
<ul>
<li>Bachelor&rsquo;s Degree with a major in social work, counseling, psychology or related human services field</li>
<li>Two years minimum experience working with children, youth and families in a home or community setting preferred</li>
<li>Valid driver&rsquo;s license with reliable and consistent access to a vehicle</li>

<p>The Case Manager gathers information from the youth and family to develop and monitor the case plan. The Case Manager role requires to pay attention to detail, time management, and communication skills to engage the family and work with other agencies and/or stakeholders.</p><p><b>We are looking for a Case Manager who must have:</b></p><ul><li>Bachelor&rsquo;s Degree, or an Associate&rsquo;s degree with two years of experience working with youth. A combination of education and appropriate experience can be substituted for degree.</li><li>Two years&rsquo; experience in youth and family case management and/or counseling. Familiarity with Juvenile Justice System and community resources preferred.</li><li>Must possess a valid Florida Driver&rsquo;s License and Auto Insurance.</li></ul><p><b>Job Duties and Responsibilities:</b></p><ul><li>Utilize and update internal electronic databases continuously and consistently with requirements, case details, etc.</li><li>Electronically document, in detail, all case and field notes within 72 hours, utilizing the Juvenile Justice Information System.</li><li>Make appropriate referrals to community agencies and follow-up within timeframe defined by Quality Improvement Standards.</li><li>Communicate with team and maintain knowledge of all available community resources, classes, and groups. Attend and facilitate client classes as required.</li><li>Use discretion to close cases successfully or unsuccessfully.</li><li>Manage daily work schedule and work independently in the field without direct supervision.</li><li>Perform all other related duties as assigned</li></ul><p><b>Job Essentials:</b></p><ul><li>Manage assigned case load of diverted youth by providing supervision to youth through individual, collateral, and family contacts.</li><li>Complete program entry paperwork on all youth within timeframe defined by Standard Operating Procedures.</li><li>Ensure that youth is complying with all sanctions and goals as outlined on their plan,</li><li>Schedule and attend school related staffing, when needed.</li><li>Maintain contact with all collateral services and Juvenile Justice Probation Officers as needed and within timeframe defined by Quality Improvement Standards.</li><li>Required to drive daily for client visits, school checks and meetings.</li></ul><p><i><b>BAYS is a drug-free workplace.
