About Homeless Children&rsquo;s Network (HCN)

Since 1992, HCN has been San Francisco&rsquo;s leading nonprofit agency for comprehensive early intervention and mental health support for underserved and historically marginalized children, youth, and families experiencing homelessness, housing instability, or living in intergenerational poverty. Our mobile therapeutic model is both accessible and scalable, enabling clinicians to meet clients directly in the community. As the hub of a Collaborative of 60+ service agencies and community-based organizations, we serve over 2,500 clients each year, reaching those most reluctant to trust conventional mental health systems. Central to our work is our Afri-centric mental health and wellness model. This community-defined approach addresses health inequities and the persistent cycle of racism and trauma over generations. This model not only uplifts the Black/African American communities but also strengthens the behavioral health outcomes of all San Franciscans, fostering resilience, healing, and lasting change.

FAMILY WELLNESS PROGRAM OVERVIEW: 
The Family Wellness Program is a community-based initiative funded by the San Francisco Department of Children, Youth and Their Families (DCYF) designed to support the emotional, social, and mental well-being of K&ndash;12 youth and their families. The program provides accessible, culturally responsive services in a welcoming, family-centered environment.

Core services include short-term individual and family mental health support, wellness and enrichment programming for youth, family recreation and engagement activities, and case management and resource navigation. The program operates primarily through weekly on-site programming and quarterly events, with a strong emphasis on reducing barriers to care and increasing access to services. 
POSITION SUMMARY: 
HCN provides resources and referrals, which are designed to enhance families&rsquo; sense of connection to the communities in which they live and ensure that the changing needs of the child and family are met. Our work with families is strengths-based, and both respectful of and responsive to cultural and linguistic differences in various communities.

The Case Manager is responsible for providing housing stability plans for at least 18 households per year. As part of this work, the Case Manager may be supporting families in:

1) searching for and securing housing,

2) increasing income and employability,

3) improving credit history and rental stability,

4) addressing behavioral health issues that negatively impact housing stability, and/or

5) accessing permanently affordable housing, including applying for placement on appropriate wait lists.

6) Weekly documentation deadlines
Duties include but are not limited to those listed above in the efforts to support families in obtaining and/or maintaining stable housing. 
REQUIRED QUALIFICATIONS:

Obtained a Bachelor&rsquo;s degree, especially in Psychology, Sociology, Social Work, or similarly related field</li>
<li>Familiar with resources and communities in San Francisco, especially those dedicated to families with low income and experiencing homelessness/housing instability</li>
<li>Charismatic and outgoing, and look forward to networking and outreaching with individuals and organizations</li>
<li>Thrive in independent work while coming alive in a community of like-minded colleagues who share similar values</li>
<li>Tenacious and able to follow through in order to best support client needs</li>
<li>Willing to travel throughout San Francisco to meet with clients, families, and community partners as safety guidelines allow</li>
<li>Preferably bilingual and bicultural Spanish/English</li></ul>

Essential Functions: 
<ul>
<li>Provide and coordinate a continuum of case management services on all levels of the child welfare system designed to achieve permanency for children within their family of origin, extended family, or a foster/adoptive family within the policies and procedures of the agency, accreditation agency and regulatory agency.</li>
<li>Perform all tasks as required in the written service plan that is based upon the comprehensive, integrated assessment being certain to engage children and families in the process.</li>
<li>Complete and submit all reports, documentation, and billing accurately and timely as required by the agency and funding sources.</li>
<li>Respond in person to client emergencies 24-hours-a-day, attend court hearings, and ACRs.</li>
<li>Ensure productivity requirements established by the Program Director or designee are met.</li>
<li>Participate in the supervisory process for guidance, direction support, and approval of decision-making, assessment, service planning, and intervention. Present cases for staffing and conduct family-child meetings.</li>
<li>Coordinate and collaborate with other service providers, schools, and health care facilities to promote the child(ren)’s well-being.</li>
<li>Communicate with supervisor, work group, and collateral agencies regarding children’s and/or family’s status.</li>
<li>Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.</li>

Position Qualifications:

<li>Master’s degree in social work or other related human services field required.</li>
<li>Minimum of one (1) year of related experience preferred.</li>
