Position Summary

The Family Consultant serves as the primary liaison between Community Connections, foster families, referral agencies, and internal stakeholders to ensure timely, appropriate, and stable placements for children in foster care. This role is responsible for coordinating placement activities, supporting foster families throughout the placement process, monitoring placement stability, and ensuring all placement-related documentation and communication are completed accurately and timely.

The Family Consultant plays a critical role in ensuring children are placed with qualified foster families while promoting positive outcomes, regulatory compliance, and exceptional customer service for both foster families and referral partners.

Essential Duties and Responsibilities

Foster Family Support

  • Serve as the primary point of contact for assigned foster families regarding placement opportunities and support needs.
  • Provide guidance and support to foster families during placement transitions.
  • Address concerns, challenges, and placement disruptions proactively and escalate issues as needed.
  • Foster positive relationships with caregivers to encourage retention and engagement.
  • Advocate for the child's best interests during court hearings, IEP meetings, staffing, and team meetings.
  • Equip foster families with referrals and resources to support to support placement stability and child success.

Case Coordination and Monitoring

  • Conduct regular home visits and out-of-home visits to monitor child safety and well-being.
  • Maintain thorough and timely documentation of case notes, home visit reports, service plans, and progress updates in accordance with state and agency standards.
  • Coordinate with internal and external stakeholders to address placement-related concerns.
  • Document all placement activities, communications, and outcomes in agency systems.
  • Participate in placement meetings and multidisciplinary team discussions.
  • Assist with placement transitions, reunifications, and discharge planning activities.

Compliance and Documentation

  • Maintain accurate and timely records of referrals, placement activities, and family communications.
  • Ensure compliance with Community Connections polices, RCCL guidelines, and RBWO standards.
  • Prepare and submit documentation to DFCS, the courts, and other stakeholders as needed.
  • Ensure that all case management data systems are continuously updated with required and relevant case details (i.e. GA Score, Extended Reach, Internal files, etc.,)
  • Support audits, reviews, and reporting requirements related to placement services.

Relationship Management

  • Build and maintain positive working relationships with DFCS personnel, referral agencies, foster families, and community partners.
  • Represent Community Connections professionally during meetings, events, and stakeholder interactions.

Crisis Intervention & On-Call Support

  • Provide weekday evening on-call coverage for assigned cases.
  • Participate in rotating on-call coverage during weekends and holidays.
  • Provide support and response during critical incidents, with guidance from the Family Consultant Supervisor when necessary.

Additional Responsibilities

  • Assist with agency initiatives aimed at improving placement stability and foster family satisfaction.
  • Attend required meetings, trainings, and professional development activities.
  • Perform other duties as assigned.

Knowledge, Skills, and Abilities

  • Knowledge of foster care, child welfare, and placement services.
  • Strong understanding of trauma-informed care principles.
  • Excellent interpersonal and relationship-building skills.
  • Strong verbal and written communication abilities.
  • Ability to assess complex situations and make sound decisions under pressure.
  • Strong organizational and time-management skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with Microsoft Office Suite and case management systems.
  • Ability to maintain confidentiality and professionalism at all times.

Education and Experience

Required

  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Child and Family Development, or a related field.
  • Minimum of two (2) years of experience in foster care, child welfare, case management, family services, or a related human services field.

Preferred

  • Knowledge of Georgia child welfare regulations and licensing requirements.
  • Experience working with medically fragile children and specialized foster care populations.

Physical Requirements

  • Ability to sit, stand, and use a computer for extended periods.
  • Ability to travel locally as needed.
  • Ability to occasionally work evenings, weekends, or on-call schedules.

Work Environment

This position operates in a professional office environment with occasional travel to foster homes, community locations, and partner agencies. The role requires flexibility to respond to placement needs outside of standard business hours when necessary.
