Family Coordinator

SUMMARY:
The Coordinator II position works with assigned families, to assess their identified needs and provide education, assistance, and advocacy in those areas. If needed, the Bridging Families Collaborative Case Manager procures short term emergency shelter services for families at scattered sites to include apartment complexes and extended Stay Hotels. This position also is the liaison between landlords and hotel management who have entered into an agreement with the agency to ensure the units are safe and address any concerns from either the client and/or landlord/hotel management.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

The specific expectations and essential duties of this position include:

  • Conducts intake procedures for St. Vincent de Paul CARES Bridging Families Collaborative Program
  • Works with outside agencies to include community outreach teams and local police departments to receive referrals and access the family’s needs
  • Abide to the guidelines of the Pinellas County Homeless Leadership Boards Coordinated Entry System
  • Maintains documentation and records related to intake files.
  • Effectively links clients with appropriate community resources and advocates for client needs.
  • Provides effective crisis management
  • Works cooperatively with members of a larger treatment or service coordination team, either internally within the agency or externally in the larger community
  • Effectively works with landlords and hotels.
  • Is available to clients at all hours required by program or contracts
  • Other duties as assigned

  • Complies with all applicable training requirements.
  • Complies with all company safety, personnel and operational policies and procedures.
  • Complies with work schedule to ensure effective operations of Agency programs.
  • Contributes positively as a member of a productive and cooperative team.
  • Performs other duties as necessary to fulfill the St. Vincent de Paul CARES Mission.

  • Able to speak, write and understand English.

  • Knowledge of the characteristics and cultural diversity with families.
  • Ability to meet deadlines and handle crisis.
  • Exceptional record keeping skills; ability to obtain and maintain accurate, detailed and confidential notes and client files specific to program requirements.
  • Excellent written and verbal skills, which include word processing, data entry and email.
  • Ability to problem solve and make decisions independently as needed.
  • Valid state driver’s license and a reliable vehicle for travel is required.
  • Ability to evaluate family needs and work in partnership with them to set realistic goals.


EDUCATION AND EXPERIENCE:
(Pending on position and if prior to hire is approved by Chief Executive Officer, a comparable amount of training, education or experience may be substituted for the minimum education qualifications)

  • Bachelor’s Degree in human service field and 2-years’ related experience.
  • Ability to work independently and as a team


GENERAL PHYSICAL DEMANDS
: These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the job. Working in an office/site requires prolonged sitting at the computer workstation; standing, bending, reaching, lifting up to 40lbs. and some driving. Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, telephones, and other office equipment. It is also required to regularly sit, speak, and listen, the employee is also required to walk, use hands and fingers to type, operate equipment, and maintain records and notes. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.


MENTAL DEMANDS:
Must handle new and diverse work problems on a daily basis. Personal maturity is an important attribute. Must be able to resolve problems, handle conflict, and make effective decisions under pressure. Must have the ability to listen objectively to people, perceive the real problem and assist in bringing issues to a successful conclusion. Must relate and interact with, volunteers, clients, contractors, visitors and employees at all levels within the Agency.


WORK ENVIRONMENT:
Environment will occasionally become noisy due to equipment operations and interactions among clients and staff. There may be the possibility of being exposed to communicable disease, possible exposure to verbal abuse or similar behavior from residents/clients. On an as needed basis, employees may be called upon to work outside of the established work schedule or work odd hours. All information associated with the Agency is confidential.
