The Family Nurse Practitioner serves as a licensed independent practitioner responsible for delivering comprehensive primary care services across the lifespan, including pediatric, adolescent, and adult populations.
This role contributes directly to Radical Health’s innovative care model, integrating clinical care with care navigation, behavioral health awareness, and family-centered support.
The Nurse Practitioner provides patient- and family-centered care in accordance with Radical Health policies, New York State scope of practice, and evidence-based clinical standards.

Work closely with care navigators and community health workers

Support patients impacted by Medicaid access barriers and social determinants of health

Help design and shape a new model of community-rooted primary care

Practice with autonomy while supported by a collaborative clinical team

Provide comprehensive primary care services including preventive care, acute care, and chronic disease management across the lifespan
-
Deliver pediatric care including well-child visits, sick visits, developmental screenings, and vaccine counseling
-
Conduct physical examinations, health assessments, and diagnostic evaluations
-
Formulate differential diagnoses and develop appropriate care plans
-
Order, interpret, and act on diagnostic tests and laboratory results
-
Prescribe medications and therapeutic interventions in accordance with scope of practice
-
Coordinate specialty referrals and ensure continuity of care
-
Maintain accurate, timely, and compliant clinical documentation in the electronic medical record
-
Deliver trauma-informed, culturally responsive, and relationship-centered care
-
Collaborate closely with Radical Health care navigators, community health workers, and program staff

Participate in care coordination efforts and multidisciplinary team collaboration
-
Support patient engagement, education, and health promotion
-
Contribute to improving access, trust, and continuity of care
-
Participate in quality improvement initiatives
-
Support the development of clinical workflows and care protocols
-
Contribute to clinical program growth, including telehealth and integrated care services
-
Serve as a thought partner in building a community-rooted care model
-
Provide continuous, comprehensive primary care services to pediatric and adult patients
-
Assess, diagnose, and manage acute and chronic medical conditions
-
Develop, implement, and evaluate patient care plans
-
Identify health risks and implement preventive care strategies
-
Educate patients and families on treatment plans, medications, and health management
-
Collaborate with supervising physicians and clinical team members as needed
-
Maintain compliance with regulatory, credentialing, and documentation requirements
Required:
-
Licensed Family Nurse Practitioner in New York State
-
Board certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
-
Minimum 5 years of clinical experience in primary care, family medicine, or community health setting
-
Experience providing care across the lifespan, including pediatric and adult patients
-
Bilingual in English and Spanish (required)

Strong diagnostic, clinical reasoning, and patient management skills
-
Experience working with Medicaid populations and diverse communities
-
Current DEA registration — required to prescribe controlled medications
Preferred:

Experience in community health centers, FQHCs, or safety-net settings
-
Experience with trauma-informed care and culturally responsive care models
-
Experience with behavioral health integration and care coordination

SALARY & BENEFITS:

Salary Range: $130,000 - $140,000
