Family Nurse Practitioner

Genesis Medical Health, PC is a progressive medical group providing Physician, Nurse Practitioner (NP), and Physician Assistant (PA) services to assisted living and nursing facilities throughout the New York area. We focus on honesty, integrity, professionalism, and exceptional care.

Our providers deliver proactive, comprehensive care with regular on-site coverage, monthly check-ups, admission exams, and timely acute visits — all aimed at improving resident outcomes and reducing avoidable hospitalizations. As we continue expanding across New York, we’re looking for dedicated clinicians who want to grow with us and help elevate the standard of care in our partner facilities.

This is a full-time, on-site position for a Family Nurse Practitioner serving residents in assisted living and nursing home settings throughout the New York City Metropolitan Area.

Responsibilities include:

• Providing primary care services and managing both chronic and acute conditions
• Conducting admission exams and routine monthly visits
• Collaborating closely with facility staff and interdisciplinary teams
• Helping reduce rehospitalizations through attentive and timely care

• Licensed Nurse Practitioner (NP) with a strong foundation in Nursing
• Experience in Primary Care and a solid understanding of general medicine
• Background in Urgent Care, long-term care, or similar settings preferred
• Strong organizational, communication, and decision-making skills
• Ability to work independently while collaborating with medical and facility teams
• Current NP licensure in the state of New York
• Experience in nursing homes, assisted living, or long-term care highly preferred
