Family Nurse Practitioner

Job Summary:

We at DHCS are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our dynamic healthcare team. We are looking for a mission-driven FNP who is ready to hit the ground running and serve underserved communities in Washington, DC. The ideal candidate will demonstrate a strong commitment to providing high-quality, patient-centered primary care across a range of settings, including geriatrics and behavioral health support. As an FNP, you will play a vital role in assessing, diagnosing and managing health conditions while emphasizing wellness, disease prevention and continuity of care for those who need it most.

Key Responsibilities

  • Conduct comprehensive primary care assessments and physical examinations.
  • Develop and implement individualized care plans based on patient needs.
  • Administer medications and perform clinical procedures (e.g., catheter care) in accordance with established standards.
  • Provide education and counseling to patients regarding health promotion, disease prevention, and medication adherence.
  • Collaborate with interdisciplinary teams to ensure holistic patient care.
  • Maintain accurate patient records using EHR your way, adhering strictly to HIPAA regulations.
  • Address occupational health concerns and provide behavioral health support within a primary care scope.

Qualifications

  • Active Nurse Practitioner license (Required).
  • Board Certification as a Family Nurse Practitioner (AANP or ANCC).
  • Bachelor’s in Nursing required; Master’s degree strongly desired.
  • Mission-Ready: A high level of readiness and passion for serving underserved and vulnerable populations.
  • Proficiency in EHR your way or similar EHR systems preferred.
  • Experience in geriatrics, occupational health, or community health is highly desirable.
  • Clear, compassionate communicator with strong patient education skills.
  • Ability to work independently and as part of a collaborative team.

Job Type: Full-time

Pay: $130,000.00 - $150,000.00 per year
