Family Nurse Practitioner

Job Overview

We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team at Mojave Complete Recovery. In this vital role, you will provide comprehensive primary care and Medication-Assisted Treatment (MAT/MOUD) services for adults with substance use disorders while promoting overall health and wellness. You will support adult patients across a wide range of clinical needs with a strong focus on integrated behavioral health and addiction medicine. Your expertise will help improve patient outcomes through thorough assessments, individualized treatment planning, and collaborative care within an integrated care model that includes behavioral health, counseling, and case management. This hybrid position includes both telehealth services and in-person patient carewithin our outpatient clinical setting, providing flexibility while maintaining high standards of patient care and accessibility for individuals seeking recovery and whole-person treatment.

Duties

Conduct detailed patient assessments, including physical examinations, vital signs, substance use evaluations, and medical history reviews to determine health status and treatment needs through both telehealth and in-person clinical encounters.

Develop and implement individualized care plans that incorporate evidence-based practices, including Medication-Assisted Treatment (MAT/MOUD) protocols and patient-centered recovery goals.

Prescribe and manage medications for both primary care conditions and substance use disorders, including buprenorphine-based treatments (e.g., Suboxone) and other MAT medications when appropriate, while monitoring patient response and safety.

Perform procedures and clinical interventions appropriate to adult primary care and outpatient behavioral health settings during in-person visits while ensuring adherence to sterile technique and safety protocols.

Utilize electronic health record (EHR) systems to document patient encounters accurately and efficiently, including MAT documentation, treatment planning, and regulatory compliance.

Provide patient education and health coaching on recovery support, relapse prevention, chronic disease management, and preventive care through both telehealth and in-person interactions.

Collaborate with behavioral health providers, counselors, case managers, and external specialists such as psychiatry, infectious disease, and primary care providers to coordinate comprehensive treatment plans.

Participate in quality improvement initiatives and program development related to substance use disorder treatment and integrated care delivery.

Manage urgent clinical needs and patient concerns, including withdrawal management assessments, medication stabilization, and infection control protocols.

Support care coordination, discharge planning, and case management activities to ensure continuity of care and long-term recovery outcomes.

Skills

Proven experience working with adult populations including individuals with substance use disorders, co-occurring behavioral health conditions, and complex chronic medical conditions.

Strong proficiency with EMR/EHR systems; familiarity with CPT coding and ICD-10 coding for accurate clinical documentation and MAT-related billing.

Knowledge of addiction medicine principles, withdrawal management, relapse prevention strategies, and harm reduction approaches.

Understanding of physiology and pharmacology relevant to adult primary care and substance use disorder treatment; ability to interpret laboratory results and diagnostic evaluations.

Experience in outpatient, behavioral health, addiction treatment, or community health environments is highly valued.

Ability to perform telehealth consultations effectively while maintaining HIPAA compliance and continuity of MAT care.

Competence in managing complex cases involving co-occurring mental health conditions, chronic disease management, and recovery-oriented treatment planning.

Excellent communication skills for patient education and interdisciplinary collaboration; commitment to delivering compassionate, stigma-free, patient-centered care.

Ability to manage high-demand clinical environments while maintaining regulatory compliance and high standards of patient safety.

Pay: $60.00 - $70.00 per hour
