Family Nurse Practitioner

Overview

We are seeking a skilled and compassionate Mobile Family Nurse Practitioner (FNP) to provide in-home medical evaluations, home health eligibility assessments, and concierge medical services. The provider will travel to patient homes to perform clinical assessments, determine eligibility for home health services based on agency and insurance guidelines, and deliver personalized medical care through concierge-style services. This role supports patients by bringing high-quality healthcare directly to their homes while ensuring compliance with healthcare regulations and agency standards.

  • Travel to patient homes to conduct comprehensive medical evaluations and physical assessments.
  • Determine patient eligibility for home health services according to Medicare, insurance, and agency guidelines.
  • Perform Face-to-Face evaluations required for home health certification when applicable.
  • Provide concierge-style medical services, including patient consultations, wellness checks, and follow-up visits in the home setting.
  • Review medical history, medications, and current health conditions.
  • Evaluate functional status, mobility limitations, and home safety needs.
  • Provide patient education, preventative care guidance, and health management support.
  • Coordinate care with physicians, home health agencies, case managers, and other healthcare providers.
  • Document patient encounters accurately and maintain timely clinical documentation in compliance with regulatory standards.
  • Ensure compliance with HIPAA, Medicare guidelines, and agency policies.
  • Communicate clinical findings and recommendations to referring providers and care teams.

Qualifications

  • Active Family Nurse Practitioner (FNP) license in the state of practice.
  • Board Certification (AANP or ANCC).
  • DEA license (preferred depending on scope of services).
  • Minimum 1–2 years of clinical experience preferred.
  • Experience in home health, primary care, geriatrics, wound care, or internal medicine is highly preferred.
  • Ability to work independently in a mobile healthcare environment.
  • Valid driver’s license and reliable transportation.

Pay: $120.00 - $200.00 per hour
