Virtue Mobile Care is a physician-led organization providing relationship-centered, proactive primary care in the home for Medicare beneficiaries.

Position Summary

The Nurse Practitioner will conduct in-home medical visits across your assigned area, including routine follow-ups, chronic care management, acute visits, and patient education.

Key Responsibilities

· Patient assessments: Conduct physical examinations, evaluate patient conditions, and interpret diagnostic tests like lab results and X-rays.

· Treatment and diagnosis: Diagnose and treat both acute and chronic illnesses, and prescribe medications or other forms of treatment.

· Patient education: Counsel patients and their families on their health conditions, prescribed medications, and preventative measures to promote overall wellness.

· Care coordination: Collaborate with physicians, case managers, and other healthcare professionals to create comprehensive patient care plans.

· Administrative tasks: Record patient findings, manage electronic health records, and complete necessary documentation.

· Continuing education: Attend staff education and trainings as offered to ensure organizational goals of providing the highest quality of care, utilizing best practices, and reducing hospitalizations are maintained.

· Adaptability, organization, and problem-solving skills to manage care in varied and unpredictable settings.

· Familiarity with electronic health records and telehealth platforms is often essential.

Work Environment

· Travel to patients' homes, assisted living facilities, community centers, or other designated sites.

· Often involves working with minimal direct supervision and requires strong time-management skills.

Required Qualifications (California)

  • Active California Nurse Practitioner License
  • NP Furnishing Number (or eligibility to obtain)
  • DEA Registration (or eligibility to obtain prior to patient care)
  • California Schedule II Controlled Substance CE if prescribing controlled substances
  • Valid California Driver’s License and reliable transportation

AB 890 Scope of Practice: Role supports Standardized Procedures or independent practice if 4,600-hour transition requirement is met.

Preferred Qualifications:

  • Fluency in Vietnamese, Mandarin and Cantonese to serve diverse patient populations

Schedule

  • Primarily Monday–Friday daytime
  • Weekend availability when clinically appropriate
  • No routine night call or 24/7 coverage expectations

Compensation

· This position follows a revenue-share compensation model. Nurse Practitioners receive 60% of Medicare Part B Fee-For-Service revenue.

· Typical annual earnings range: $80,000 – $140,000, depending on visit volume and scheduling preferences.

· Mileage reimbursed at the current federal IRS rate ($0.70 per mile, updated annually). Credentialing, billing, scheduling coordination, and administrative support provided.

· Optional healthcare benefits available.

Work Location: On the road

Pay: $75.00 - $100.00 per hour

Expected hours: 2 – 40 per week
