Family Nurse Practitioner

Job Overview
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. Duties

• Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to determine health status and needs.
• Develop and implement personalized treatment plans encompassing medication management, lifestyle counseling, and health promotion strategies.
• Manage acute conditions like pain episodes, infections.
• Utilize electronic health record (EHR) systems such as Athena, Epic or eClinicalWorks for accurate documentation of patient encounters, diagnoses (ICD-10/ICD-9 coding), and treatment plans.
• Collaborate with specialists in areas such as gastroenterology, infectious disease care, behavioral health, hospice & palliative medicine, and senior care to coordinate comprehensive patient management.
• Engage in telehealth consultations to extend care access and support patients remotely while maintaining HIPAA compliance.
• Provide patient education on topics like family planning, nutrition, medication administration, and chronic disease management to empower individuals in their health journeys.
• Support discharge planning, case management activities, and utilization review processes to optimize resource use and ensure continuity of care across inpatient and outpatient settings.
