Family Nurse Practitioner

We are a growing mobile medical practice providing high-quality, patient-centered care to geriatric patients in group homes and private residences. Our mission is to improve access to care for homebound individuals through compassionate, comprehensive services.

Position Summary:
We are seeking a compassionate and experienced Nurse Practitioner (NP) with a background in geriatric care and house call services. This role is ideal for a provider who is comfortable working independently and managing complex chronic conditions in a home-based setting.

Key Responsibilities:

• Perform in-home visits for geriatric patients
• Conduct assessments, diagnose, and develop treatment plans
• Manage chronic conditions (e.g., hypertension, diabetes, dementia)
• Collaborate with caregivers and group home staff
• Prescribe medications and monitor outcomes
• Complete accurate and timely documentation

Qualifications:

• Active Nurse Practitioner license in Arizona
• Independent Physician Assistant
• Board certified (PA, FNP, AGNP, or GNP preferred)
• Experience in geriatrics and/or home-based care
• House call experience strongly preferred
• Strong clinical and independent decision-making skills
• Reliable transportation and valid driver’s license
