Responsibilities
- Conduct comprehensive patient assessments (in-person and telehealth) thorough patient assessments, including physical examinations, and medical history reviews to determine health status and needs.
- Develop and implement individualized care plans for medical weight loss, men and women hormonal replacement therapy and general wellness optimization.
- Administer medications and injections, including specialized treatments
- Utilize electronic health record (EHR) systems to document patient encounters accurately and efficiently.
- Collaborate with multidisciplinary teams including specialists to coordinate comprehensive patient care.
- Interpret Labs and adjust treatment protocols accordingly.
- Engage in telehealth consultations to extend access to quality healthcare services beyond traditional settings.
- Participate in clinical research initiatives or quality improvement projects to advance evidence-based practices.
- Provide on going education on health coaching, medication management, nutrition, and discharge planning to empower patients in their wellness journeys.
Skills
- Proficiency with EMR/EHR systems
- Dual certification (Family and PMHNP) is a plus
- Experience with a variety of medical procedures including IV infusion, venipuncture, and sterile technique.
- Knowledge of acute care settings such as ICU or Level I Trauma Centers; hospital medicine experience is highly valued.
- Excellent communication skills for triage calls, patient education, and interdisciplinary teamwork across outpatient clinics or assisted living facilities.
- Critical thinking skills for diagnostic evaluation and utilization review within fast-paced environments like emergency rooms or urgent care clinics.
