Family Nurse Practitioner

Job Summary
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our dedicated healthcare team. In this role, you will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. Your expertise will support preventive health, acute and chronic disease management, and health education. You will work collaboratively within a multidisciplinary team to deliver high-quality, patient-centered care in various settings such as outpatient clinics, urgent care centers, and telehealth platforms. Responsibilities

  • Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans.
  • Manage acute illnesses and chronic conditions with evidence-based treatment protocols, including medication administration and IV infusion therapies.
  • Perform procedures such as suturing, dermal fillers, spinal taps, catheterizations, and sterile processing techniques with precision and adherence to safety standards.
  • Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Cerner for accurate documentation of patient encounters, medical records, and coding including CPT, ICD-10/ICD-9.
  • Provide health coaching and education tailored to diverse populations including pediatric, geriatric, behavioral health, hospice & palliative medicine, and individuals with developmental disabilities or memory care needs.
  • Participate in clinical research initiatives and quality improvement projects to enhance patient outcomes.
  • Collaborate with specialists in gastroenterology, infectious disease care, cardiology (including cardiac catheterization), ophthalmology, and other disciplines as needed for comprehensive patient management.
  • Assist in triage assessments via telehealth or in-person visits to prioritize urgent cases such as acute pain management or airway management emergencies.
  • Support discharge planning, case management, utilization review, and working within Workers' Compensation law or occupational health protocols when applicable.

Experience

  • Proven experience as a Family Nurse Practitioner or equivalent role in diverse healthcare settings such as hospitals (including Level I Trauma Centers), clinics, urgent care centers, or nursing homes.
  • Demonstrated proficiency with EMR/EHR systems like Epic, Athenahealth, Cerner or similar platforms for medical documentation and coding accuracy.
  • Hands-on experience performing procedures such as suturing, dermal fillers (esthetic laser treatment), spinal taps, catheterization (including Foley catheters), phlebotomy/venipuncture, tube feeding management, and sterile processing techniques.
  • Background working with pediatric patients (including toddler care), geriatrics (memory care/dementia care), behavioral health cases or neonatal ventilator management is highly desirable.
  • Critical care experience in ICU or emergency room environments is preferred; familiarity with Level II trauma centers or Level I trauma center protocols is a plus.
  • Knowledge of CPT coding and ICD coding standards for accurate billing; experience with diagnostic evaluation including sonography or laboratory testing is advantageous.
  • Experience providing telehealth services utilizing HIPAA-compliant platforms; working within hospital medicine or post-acute care settings enhances your candidacy.
  • Strong understanding of anatomy knowledge, physiology principles related to acute care scenarios such as gastric lavage or ventilator management; familiarity with infection control practices is essential. Pay: $112,687.21 - $135,709.33 per year
