Family Nurse Practitioner

Job Summary

We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP), advanced practice registered nurse (APRN) to join our team. The FNP will provide family-focused healthcare services to patients across the lifespan, from infants to seniors, providing primary healthcare services such as comprehensive care, including physical exams, diagnostic testing, treatment planning, and health education to promote wellness and disease prevention. In this role, you will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. Your expertise will support health promotion, disease prevention, acute and chronic illness management, and patient education. You will work collaboratively with physicians, specialists, and support staff to deliver high-quality, patient-centered care in a fast-paced environment. Duties

  • Conduct thorough patient assessments including physical examinations, vital signs, and medical history reviews to determine health status and formulate care plans.
  • Diagnose and manage acute illnesses and chronic conditions utilizing evidence-based practices aligned with current clinical guidelines.
  • Administer treatments such as injections, suturing, IV infusions, etc depending on experience with procedure.
  • Perform diagnostic evaluations including laboratory specimen collection/processing, EKG, PFTs
  • Prescribe medications accurately using CPT coding standards while ensuring compliance with HIPAA regulations for patient privacy and medical documentation.
  • Provide patient education on disease management, medication usage, lifestyle modifications, and preventive health strategies tailored to individual needs.
  • Collaborate with specialists in cardiology, pulmonology, gastroenterology, infectious disease care, geriatrics, behavioral health, hospice & palliative medicine, occupational health, and other fields to coordinate comprehensive care plans.
  • Utilize EMR systems such as Nextgen for documentation of patient encounters, treatment plans, and follow-up care.
  • Engage in telehealth consultations to expand access to care while maintaining high standards of clinical quality.
  • Participate in clinical research initiatives or quality improvement projects aimed at advancing practice standards and patient safety.

Skills

  • Proven experience working with diverse populations including children, seniors in assisted living or nursing home settings, patients with developmental disabilities or memory care needs.
  • Strong proficiency in EMR/EHR systems such as Epic or Athenahealth; familiarity with ICD-9/10 coding and CPT coding for accurate documentation.
  • Skilled in performing procedures like suturing, catheterization (including urinary catheters), phlebotomy/venipuncture, sterile processing techniques, airway management including ventilator use.
  • Experience managing acute pain cases using advanced pain management protocols
  • Knowledge of infection control practices; experience with infection prevention measures in outpatient settings.
  • Ability to perform triage effectively in urgent care or emergency room contexts; adept at assessing vital signs and conducting comprehensive patient assessments rapidly.
  • Excellent communication skills for teaching patients about their health conditions; providing health coaching tailored to individual goals.
  • Strong understanding of medical terminology related to internal medicine, pediatrics, geriatrics, pharmacology (including psychopharmacology), and physiology knowledge essential for accurate diagnosis and treatment planning. Requirements and Credentials:

  • Education: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Licensure: Active Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license in good standing within the state.
  • Certification: Board Certified (FNP-BC or FNP-C) by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).
  • Certification: Basic Life Support (BLS) and/or Advanced Cardiovascular Life Support (ACLS) certification.
  • Experience: Previous experience in primary care or family medicine is preferred but not always required for new graduates.
  • Skills: Strong diagnostic, clinical decision-making, and communication skills.
