Family Nurse Practitioner

JOB SUMMARY

A nurse practitioner is an advanced practice nurse that helps with all aspects of patient care, including diagnosis, treatments, and consultations. They may work in both inpatient and outpatient situations and can perform independently or as part of a treatment team. Generally, nurse practitioners perform the important task of educating patients about preventative care and the prescribed treatments. They may also conduct physicals, order tests and serve as a patient's primary healthcare provider. Some nurse practitioners are also able to prescribe medications. While nurse practitioners perform independently in the community health center, it is the expectation that the nurse practitioner assesses, diagnoses, and provides appropriate treatment to patients, and when necessary, consult with the physician(s) at the health center who have the delegated prescriptive authority for that nurse practitioner. The nurse practitioner operates under that physician’s prescriptive license and must provide the minimum expected charts to be reviewed by the delegated physician on a regular basis.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Patient Visits and Patient Panel

· Minimum encounters: average 20 per day or minimum 400 per month

· Minimum patient panel: 750 – 1,200 patients to manage

Clinical

· Blood Sample Collection

· Body Fluid Sample Collection

· Confer with medical and dental providers on patient conditions

· Coordination of Hospitalization if necessary

· CPR

· EKG (ECG)

· Function as the patient’s primary care provider at the health center.

· Injections

· Injections

· Inventory of Medications (Medication Management)

· Laboratory & radiology testing

· Maintain clinical records & reports

· Management of Patient Requests & Complaints

· Medical Record Documentation completely and within the scope of specialty training

· Patient physical exam

· Perform other duties within the skill level and with additional training when necessary.

· Perform RN skills and duties when necessary

· Plan of Care: Primary Care according to specialty; Complete medical history; Medical examination and diagnosis; Treatment and/or referral.

· Prescribe appropriate medications

· Preventive health education

· Referrals to specialist if necessary, and follow up after the specialty visit to ensure the patient

· Supervise medical assistants, LVNs and RNs if needed

· Triage patients for urgent or emergency care

· Vital Signs (height, weight, blood pressure)

Clinical Quality, Infection Control, Case Management, JC/PCMH

· Direct policy writing and implementation of infection control activities and compliance

· Review care coordination activities and document activities as case management.

· Provide clinical case management.

· Direct policy writing and implementation of clinical quality measures, data gathering and performance.

· Coordinate and implement activities to secure and maintain PCMH / Joint Commission accreditation and recognition.

Administrative

· Chart Organization, Retrieval, Filing

· Clinical protocols development, writing, implementation, compliance

· Coding with appropriate diagnosis and level of service coding (ICD9 or ICD10 and CPT)

· Collaboration with other department staff

· Complete encounter forms on each patient

· Department budget management

· Department strategic planning

· Ensure patients are contacted to receive regular primary and preventive visits on time.

· Equipment & supply ordering & inventory

· Grant application submissions

· Nutritional health education

· Patient Appointment Scheduling

· Patient Education

· Phone Calls, Customer Service

· Refer to behavioral health

· Refer to dental department for exam

· Serve as after-hours provider on-call when necessary

· Serve as preceptor when necessary

· Use of charting within the electronic health record

· Volunteer to participate in company sponsored health fairs

· Assist in training staff, supervisors, and co-workers in various business operations.

· Comply with Employee Handbook Topics

Compliance Program

· Participate in activities related to Compliance, HRSA, Joint Commission, Primary Care Medical Home and other business operations.

EVALUATION:

The evaluation of work performance is ongoing and will be performed by the Chief Medical Officer. It will include the specific duties and responsibilities of this position description plus

employee attitude, general working behavior, and ability to interact constructively with the clinical and administrative staff. Formal evaluations will normally be performed annually; but may be initiated at other times by the Chief Medical Officer or CEO.

QUALIFICATION REQUIREMENTS:

Must be able to read, write, and speak English. Previous experience in operation of office equipment: Personal Computer, copier, FAX, scanner, postage machine, printers, phone switchboards and other auxiliary computer equipment.

EDUCATION AND/OR EXPERIENCE:

· Graduate of acceptable Type A NP program, recognized by Board of Medicine, State Medical Society & American Medical Association

· Licensed to practice as a Registered Nurse and TX NP license

· Read/Write/Speak English, Spanish a plus; High school level math, computer usage

· CPR Certificate, BLS Certificate

Pay: $105,000.00 - $111,000.00 per year
