Job Summary
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. Duties
- Conduct thorough physical examinations and health assessments for patients across various age groups, utilizing advanced clinical evaluation techniques.
- Diagnose and manage acute and chronic illnesses, employing evidence-based treatment plans tailored to individual patient needs.
- Administer medications, including injections and IV infusions, following proper protocols and safety standards.
- Perform specialized procedures such as suturing, dermal fillers, spinal taps, and esthetic laser treatments when appropriate.
- Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, and Cerner to document patient encounters accurately and efficiently.
- Coordinate care with specialists in gastroenterology, cardiology, infectious disease care, behavioral health, and other disciplines to optimize patient outcomes.
- Engage in telehealth consultations to expand access to care while maintaining compliance with HIPAA regulations.
- Participate in clinical research initiatives and quality improvement projects to advance evidence-based practice standards.
- Provide patient education on topics such as family planning, nutrition, memory care, hospice & palliative medicine, and senior care.
- Support discharge planning, case management, utilization review, and working with individuals with disabilities or developmental disabilities.
Requirements
- Valid Family Nurse Practitioner license with current certification from the American Association of Nurse Practitioners (AANP) or equivalent.
- Proven experience in primary care settings with proficiency in medical office operations and EMR/EHR systems such as Epic or Athenahealth.
- Demonstrated expertise in procedures including suturing, phlebotomy, catheterization, venipuncture, sterile processing techniques, and basic life support (BLS).
- Experience working with diverse populations including pediatrics (including neonatal ventilator management), geriatrics (memory care), hospice & palliative medicine patients, and individuals with developmental disabilities.
- Critical care experience in ICU or Level I/II trauma center environments is highly desirable.
- Knowledge of CPT coding and ICD coding systems (ICD-9/10) for accurate documentation and billing purposes.
- Strong understanding of infection control practices and aseptic techniques across various clinical procedures.
- Excellent communication skills with the ability to triage effectively during urgent or emergent situations in outpatient or inpatient settings.
- Ability to collaborate seamlessly within multidisciplinary teams across hospital medicine, emergency medicine, occupational health, home care services, and outpatient clinics.
