Family Nurse Practitioner

Job Summary
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. Responsibilities

• Conduct thorough physical examinations and health assessments for patients of all ages, including pediatrics, geriatrics, and special populations such as assisted living residents and individuals with developmental disabilities.
• Manage acute and chronic conditions utilizing evidence-based practices, including pain management, infectious disease care, geriatrics, and behavioral health.
• Perform procedures such as suturing, I&D with precision and adherence to sterile techniques.
• Collaborate with physicians and specialists to coordinate patient care plans, discharge planning, case management, and utilization review.
• Utilize electronic health record (EHR) systems for accurate medical documentation, medication administration records, CPT coding, ICD-10 coding, and compliance with HIPAA regulations.
• Provide patient education on health coaching topics including nutrition, medication management, family planning, and post-acute care strategies.
• Participate in telehealth consultations to extend access to quality care across various settings.

Experience

• Proven experience working in diverse clinical environments such as hospital medicine units—including ER, ICU or Level I/II trauma centers—and/or outpatient clinics.
• Demonstrated expertise in managing complex cases involving acute pain management, infectious diseases, geriatrics, pediatric care, hospice & palliative medicine, or behavioral health.
• Hands-on experience performing procedures like venipuncture, Pap smears, phlebotomy, and laboratory specimen collection/processing.
• Prior involvement in clinical research or teaching hospital settings is a plus.
• Strong knowledge of physiology and anatomy to support diagnostic evaluation and treatment planning across specialties.
