Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in primary care and be committed to providing high-quality patient care across diverse populations. This role involves working collaboratively with physicians and other healthcare professionals to deliver comprehensive health services, including preventive care, acute care, and chronic disease management.
Responsibilities
- Conduct thorough physical examinations and patient assessments to determine health status.
- Develop and implement individualized care plans based on patient needs.
- Perform diagnostic evaluations, including ordering and interpreting lab tests and imaging studies.
- Administer medications, injections, IV infusions, and other treatments as necessary.
- Provide patient education on health maintenance, disease prevention, and management of chronic conditions.
- Collaborate with healthcare teams in various settings such as outpatient clinics, hospitals, and assisted living facilities.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Participate in clinical research initiatives as needed.
- Engage in telehealth consultations to expand access to care for patients.
- Manage acute pain management strategies for patients requiring specialized care.
Qualifications
- Current certification as a Family Nurse Practitioner (FNP) with an active state license and medicare number.
- Experience in primary care settings; EMR systems is preferred
- Background in critical care, emergency medicine, or urgent care is advantageous.
- Knowledge of ICD-9/ICD-10 coding for accurate medical documentation.
- Experience working with diverse populations including geriatrics, and individuals with developmental disabilities.
- Strong understanding of medical terminology, anatomy knowledge, physiology knowledge, and infection control practices.
- Ability to provide compassionate patient-centered care while managing complex cases including hospice & palliative medicine and behavioral health issues.

Pay: $55.00 - $70.00 per hour
