Job Overview
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary care services, perform clinical assessments, and develop personalized treatment plans to promote optimal health outcomes for patients across all age groups. Your expertise will help bridge the gap between acute and chronic care, ensuring patients receive timely, effective, and empathetic medical attention in a variety of settings, including outpatient clinics, urgent care, and assisted living facilities. Responsibilities
- Conduct thorough physical examinations and patient assessments, including vital signs, medical history review, and diagnostic evaluations.
- Manage acute and chronic conditions utilizing evidence-based practices across specialties such as internal medicine, geriatrics, pediatrics, behavioral health, and emergency medicine.
- Administer injections, including immunizations and Botox or dermal fillers for aesthetic treatments, as well as IV infusions and catheterizations when necessary.
- Perform procedures such as suturing, spinal taps, phlebotomy, tube feeding management, and sterile processing techniques following aseptic protocols.
- Utilize electronic health record (EHR) systems like Epic or eClinicalWorks to document patient encounters accurately while ensuring compliance with HIPAA regulations.
- Collaborate with specialists in areas like gastroenterology, infectious disease care, hospice & palliative medicine, and occupational health to coordinate comprehensive patient care plans.
- Participate in telehealth consultations and triage services to provide accessible healthcare solutions across various platforms.
- Assist with discharge planning, case management, utilization review, and follow-up care to optimize patient outcomes in inpatient or outpatient settings.
Experience
- Valid Family Nurse Practitioner license with current certification from an accredited program.
- Proven clinical experience in primary care settings such as hospitals, nursing homes, urgent care clinics, or outpatient practices.
- Hands-on experience with EMR/EHR systems like Cerner or Athenahealth to ensure seamless documentation and patient data management.
- Demonstrated proficiency in performing procedures including suturing, venipuncture (phlebotomy), catheter care, and injection administration.
- Background working with diverse populations including pediatrics (including toddler and neonatal care), geriatrics (memory care & dementia), individuals with disabilities or developmental disabilities.
- Critical care experience in ICU or Level I/II trauma centers is highly desirable; familiarity with emergency medicine protocols is a plus.
- Knowledge of CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-9/10 for accurate billing and documentation purposes.
- Experience in specialized areas such as gastrology, pain management, behavioral health, occupational health or post-acute care is advantageous. Pay: $125,000.00 - $140,000.00 per year
