We are currently seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our growing team. This is an excellent opportunity for a provider who values relationship-based medicine, teamwork, and long-term professional growth while maintaining strong work-life balance.

Position Overview

In this role, you will provide comprehensive outpatient primary care services across a diverse patient population while working alongside experienced physicians and advanced practice providers. You will evaluate, diagnose, and treat both acute and chronic medical conditions, develop individualized treatment plans, and help improve the overall health and well-being of the patients we serve.

The Practice

Provider support includes:

  • 20–25 patients per day
  • 1–2 Medical Assistants per provider
  • Dedicated referral coordinator
  • Care coordination support
  • Prior authorization support staff
  • Established patient base with strong community presence

Our Livingston provider team includes physicians in Pediatrics, Family Medicine, and Internal Medicine along with advanced practice providers, creating a collaborative and supportive clinical environment.

Schedule & Work-Life Balance

  • Outpatient-only primary care
  • No weekends
  • No hospital work
  • Shared light call schedule (approximately every 8 weeks)
  • Average call volume approximately 5 calls per week while on call

Flexible scheduling options are available, including the possibility of a 4-day work week with adjusted compensation.

Compensation & Benefits

  • Salary range: $120,000 – $135,000+ depending on experience
  • Productivity and growth opportunities available
  • $5,000 sign-on bonus
  • Up to $5,000 relocation assistance
  • 25 days PTO

Responsibilities

  • Conduct comprehensive patient evaluations, histories, and physical examinations across all age groups
  • Diagnose and manage acute and chronic medical conditions
  • Develop and implement individualized treatment plans
  • Prescribe medications and monitor treatment response
  • Provide preventive care and chronic disease management
  • Coordinate referrals to specialists and ancillary services as needed
  • Perform office-based procedures within scope of practice
  • Utilize EMR/EHR systems for accurate documentation, coding, and patient communication
  • Collaborate with physicians, nursing staff, care coordinators, and administrative team members to optimize patient outcomes
  • Provide patient education and counseling to support long-term health goals

Qualifications & Experience

  • Current Texas Family Nurse Practitioner license
  • Current board certification from an accredited NP program
  • Strong outpatient primary care experience preferred
  • Experience with EMR/EHR systems such as Athenahealth, Epic, or eClinicalWorks preferred
  • Strong communication and interpersonal skills
  • Ability to work independently while collaborating effectively within a team environment
  • Experience managing diverse patient populations across the lifespan is highly valued

Additional experience in urgent care, geriatrics, pediatrics, chronic disease management, preventive care, behavioral health, telehealth, or rural/community medicine is a plus.
