We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary care services, perform detailed patient assessments, and develop personalized treatment plans to promote optimal health outcomes. Your expertise will support diverse patient populations across various settings, including outpatient clinics, urgent care, and specialized care units. This role offers an exciting opportunity to make a meaningful difference in patients’ lives through proactive health management, education, and compassionate care delivery.

Responsibilities

  • Conduct thorough physical examinations and patient assessments, including vital signs, medical history review, and physiological evaluations.
  • Diagnose and manage acute and chronic illnesses across age groups, utilizing clinical judgment and evidence-based practices.
  • Administer injections, including immunizations and dermal fillers, perform suturing, and assist with procedures such as spinal taps or catheterizations when necessary.
  • Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks to document patient encounters accurately and efficiently.
  • Provide health coaching, patient education on disease prevention, medication management, and lifestyle modifications tailored to individual needs.
  • Collaborate with specialists in areas such as gastroenterology, infectious disease care, behavioral health, geriatrics, or pediatric medicine to coordinate comprehensive care plans.
  • Participate in telehealth consultations and triage services to extend accessible care beyond traditional settings.
  • Ensure compliance with HIPAA regulations and maintain the highest standards of infection control and sterile processing during procedures.

Experience

  • Proven experience working as a Nurse Practitioner in diverse clinical environments such as hospital medicine, urgent care, outpatient clinics, or assisted living facilities.
  • Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for medical documentation and coding (CPT, ICD-9/10).
  • Hands-on experience with procedures such as IV infusion, phlebotomy, venipuncture, sterile technique for suturing or catheter care.
  • Critical care experience in settings like ICU or Level I/II trauma centers is highly desirable.
  • Background in managing complex cases involving acute pain management, hospice & palliative medicine, or post-acute care is advantageous.

Pay: $100,000.00 - $120,000.00 per year
