C3Cares is seeking a mission-driven, compassionate, and innovative Family Nurse Practitioner (FNP) to join our team part-time. We are redefining healthcare delivery by embedding high-quality primary care directly within public housing communities across Washington, DC.

Position Overview

The Family Nurse Practitioner will provide comprehensive, patient-centered primary care across the lifespan, including preventive care, chronic disease management, acute care, health education, and care coordination.

Key Responsibilities

· Deliver high-quality primary care services for children and adults.

· Diagnose, treat, and manage acute and chronic conditions.

· Provide preventive screenings, immunizations, and health education.

· Address social determinants of health in partnership with community resources.

· Engage patients in shared decision-making.

· Document care in EHR and contribute to quality improvement initiatives.

· Support population health and care gap closure efforts.

Qualifications

· MSN, DNP, or equivalent advanced nursing degree.

· Active, unrestricted DC Family Nurse Practitioner license.

· ANCC or AANP certification.

· 2–5 years of NP clinical experience preferred, new graduates welcome to apply

· Experience in community health or Medicaid populations strongly preferred.

· Proficiency with EHR systems.

Location & Schedule

Part-time, based in C3Cares place-based clinics within DC public housing communities.

Telehealth days may be considered as part of the provider schedule following onboarding and based on clinical, operational, and performance needs.
