Revive Family Care & Wellness is a growing integrative family medicine clinic in Caddo Mills, Texas, providing holistic, patient-centered care for both children and adults. We blend traditional primary care with functional and lifestyle medicine, offering services such as hormone replacement therapy (HRT), GLP-1-based medical weight loss, IV nutrient therapy, and select aesthetic treatments. Our mission is to support long-term wellness by addressing the root cause of illness in a compassionate and collaborative environment.

Position Overview:

We are seeking a Family Nurse Practitioner (FNP) to join our growing clinical team in either a full-time or part-time capacity. We offer a flexible schedule and are looking for a provider who enjoys family medicine, values meaningful patient relationships, and is excited about integrative approaches to healthcare. This role includes shared rotating weekend on-call coverage with minimal volume and phone triage only.

The ideal candidate will have a foundation in primary care, feel comfortable seeing pediatric and adult patients, and have an interest in functional and lifestyle medicine. One or more years of primary care experience is preferred; however, new graduates are welcome to apply and will be supported with mentorship and training.

Responsibilities:

• Deliver comprehensive care to pediatric and adult patients
• Perform wellness visits, acute care visits, and chronic disease management
• Incorporate integrative and lifestyle medicine approaches into patient care
• Participate in rotating weekend on-call coverage (remote phone triage only)
• Collaborate with a supportive and patient-focused care team
• Maintain accurate and timely medical documentation

Qualifications:

• Active Texas APRN license with FNP certification (or in process)
• Primary care experience preferred but not required
• Comfortable treating pediatric patients
• Interest or experience in functional/integrative medicine
• Bonus experience in HRT, GLP-1 therapy, IV therapy, or aesthetic injectables
• New graduates encouraged to apply
