Pathcare is seeking a compassionate and motivated Family Nurse Practitioner (FNP) to join our growing healthcare team. This is an in-person/on-site position for providers working from our clinical office location. While patient visits are conducted virtually via telehealth, providers are expected to work onsite

Providers will conduct virtual patient evaluations, review medical histories, manage ongoing care plans, complete required documentation, and collaborate with interdisciplinary teams to support high-quality patient outcomes.

Duties

Conduct telehealth patient evaluations and health assessments via phone or video visits from the office location

Review medical histories, medications, and ongoing treatment needs

Develop and manage personalized care plans for patients with acute and chronic conditions

Complete documentation accurately using EMR/EHR systems

Coordinate care with physicians, home care agencies, specialists, and interdisciplinary teams

Provide patient education, follow-up recommendations, and preventive care guidance

Assist with care plan renewals, home care evaluations, and related documentation

Maintain timely charting and compliance with clinical protocols and regulatory standards

Requirements

NPI Must be medicaid enrolled

Active Family Nurse Practitioner (FNP) license and current state registration

Experience in primary care, geriatrics, home care, telehealth, or chronic care management preferred

Comfortable conducting patient visits virtually through telehealth platforms in an in-office setting

Strong communication and patient engagement skills

Experience using EMR/EHR systems such as Epic, Athenahealth, eClinicalWorks, or similar platforms
