JOB SUMMARY

Provides comprehensive advanced practice care in a Public Health Clinic. Operates within the scope of practice designated by the practitioner’s credentials. We are looking for a dedicated and adaptable individual to join our team in a part-time role (2 days per week), with the flexibility and potential to transition into a full-time position. This is an excellent opportunity for someone seeking a long-term role with room to grow as patient demand and community need evolves.

The successful candidate will support daily operations, contribute to ongoing projects, and take on increasing patient case load over time. As the role develops, there will be opportunities for expanded hours and deeper involvement in strategic initiatives. ESSENTIAL JOB FUNCTIONS

The following duty statements are illustrative of the essential functions of the job and do not include other non-essential or marginal duties that may be required.

  • Provide primary and preventive care for patients of all ages in alignment with public health objectives.

  • Serve diverse populations, including uninsured and underinsured individuals, with a strong focus on culturally competent care to diagnose, treat, and manage acute illnesses and chronic diseases (e.g., hypertension, diabetes, asthma, infectious diseases).

  • Orders and/or performs appropriate diagnostic and screening tests and collects additional data for evaluation of illness, utilizing internal lab process when appropriate.

  • Provides managed patient care by developing and implementing individual treatment plans including: prescribing/dispensing medications; providing appropriate referrals; ensuring education, counseling, and follow-up with other professional and community agencies and documents care through established protocols and policies.

  • Engages in interdisciplinary collaboration with the Certified Nurse Midwife to ensure patients whose needs exceed midwifery scope receive appropriate referral and follow-up care within the clinic.

  • Collaborates with physicians, behavioral health providers, social workers, and public health nurses to provide integrated, wraparound care ensures appropriate referral pathways are followed, including both intra-organizational resources and external community providers, to optimize patient outcomes.

  • Integrates knowledge of clients’ needs and medical interventions to design care plans and take actions consistent with current standards of practice, licensing laws, and established policies and procedures.

  • Conduct virtual and in-person visits, using telehealth to expand reach and reduce barriers to care.

  • Support public health initiatives such as immunizations, disease screenings, reproductive health, and health education with participation in community outreach, mobile health efforts, and health promotion campaigns.

  • Maintains professional collaboration with the Medical Director, Director of Clinical Services, Clinic Manager, and relevant leadership to ensure alignment with clinical protocols and organizational objectives

  • Effectively uses electronic health information retrieval systems, data-collection processes, and technology applications for patient’s records, fiscal management, tracking, reporting, and maintaining information for individuals, populations and/or program needs to assist with program development, grant-funded projects, and quality improvement activities as needed.

  • Assists with orientation of new nursing to clinic structure, medical and administrative support staff and acts as a resource person to all staff.

  • Participates in student educational programs by orienting students to the program area, selection of client cases for demonstration/teaching of advanced practice nursing skill set, mentoring students, and providing evaluation to instructors and students.

  • Assumes responsibility for own professional development by pursuing education, participating in professional committees and work groups, and contributing to a work environment where continual improvements in practice are pursued.

NON ESSENTIAL JOB FUNCTIONS

  • Participates in emergency planning and response activities.

  • Participates in agency-wide activities that may include strategic planning, quality improvement, and/or staff development, as appropriate.

  • Completes necessary training through FEMA, including Incident Command Structure (ICS) courses.

  • When a local declaration of emergency or disaster is declared by County leadership, employees may be required to work as part of a National Incident Management System (NIMS).

  • Performs other duties as assigned.

MINIMUM QUALIFICATIONS REQUIRED

Education and Experience

  • Current licensure as a Family Nurse Practitioner to practice in the state of Colorado.

  • National certification as an FNP (AANP or ANCC).

  • Valid DEA registration and prescriptive authority.

  • At least 1–2 years of experience in primary care, community health, or public health preferred.

  • Proficient in telehealth technology and EHR systems.

  • BLS (Basic Life Support) certification.

Certificates and Licenses

  • A valid Colorado driver’s license with a responsible driving record.

  • Family Nurse Practitioner, Certified Nurse Midwife

  • Provide RN, APN and Prescriptive Authority License

Computer and Technology

  • Ability to use Google applications and Microsoft Office applications as needed to perform various job tasks.

  • Ability to use different EHR systems such as but not limited to Colorado Immunization Information System, CIIS, Quality Health Network (QHN), Allscripts, Eclinicals, etc.

PHYSICAL REQUIREMENTS and WORKING ENVIRONMENT

  • Position requires standing, stooping, bending, and stretching.

  • Stands 45% of the time, walks 45% of the time, and sits 10% of the time.

  • Work is generally confined to a very busy office with multiple tasks and interruptions.

  • Noise level is generally moderate to noisy.

  • May be exposed to communicable diseases and infections, blood and body fluids.
