Family Nurse Practitioner (FNP)

We are seeking an experienced and independent Family Nurse Practitioner (FNP) to join our growing healthcare team in Fairfax County Virginia

The ideal candidate is expected to deliver comprehensive, patient-centered primary care with a strong focus on preventive medicine, chronic disease management, and digital health innovation. This role requires clinical excellence combined with a good understanding and willingness to learn about innovative technologies to help with diagnosis and management of acute and chronic outpatient medicine.

Key Responsibilities

  • Provide comprehensive primary care services, including acute visits, chronic care and preventive care in an outpatient primary care clinic setting
  • Develop personalized preventive care plans emphasizing lifestyle modification, early diagnosis, and risk mitigation.
  • Conduct telehealth consultations to expand access to care and support continuity of treatment.
  • Leverage AI-powered scribing tools to streamline clinical documentation and improve workflow efficiency.
  • Collaborate with physicians, specialists, and multidisciplinary teams to ensure coordinated, high-quality care.
  • Educate patients on preventive health strategies, including nutrition, physical activity, immunizations, and screening protocols.
  • Ensure full compliance with HIPAA and all applicable state and federal regulations.
  • Participate in quality improvement initiatives aimed at enhancing patient outcomes and preventive care strategies.
  • Mentor and support junior clinicians, medical assistants, and care coordinators.

Qualifications & Requirements

  • Customer focused mindset with patient as the main stakeholder
  • Collaborative and pragmatic mindset to work closely with other staff
  • Board-Certified Family Nurse Practitioner (FNP-BC or AANP).
  • Active and unrestricted Virginia Nurse Practitioner license.
  • BLS required; ACLS preferred.
  • Experience in primary care and preventive medicine.
  • Proficiency with Electronic Health Records (EHR)
  • Experience with telemedicine.
  • Strong leadership, mentorship, and team collaboration skills.
  • Excellent communication, clinical judgment, and problem-solving abilities.
  • Commitment to evidence-based practice and patient-centered care.
  • A good understanding of Remote Patient Monitoring and Chronic Care Monitoring is preferred
