Salary: $140,000 – $170,000 base

COLFS Medical Clinic is seeking a compassionate and mission-driven Family Nurse Practitioner (FNP) to join our growing outpatient practice serving Escondido and San Diego. This role offers the opportunity to provide comprehensive, relationship-based care to individuals and families across all stages of life, with a strong emphasis on preventive health, chronic disease management, and whole-person care.

At COLFS Medical Clinic, we deliver integrated primary care services that include adult medicine, women’s health, prenatal support, pediatric care, STI testing, and preventive screenings. Our approach is rooted in compassionate, patient-centered care, ensuring every patient is treated with dignity, respect, and individualized attention.

This position is ideal for an FNP who values continuity of care, patient education, and community impact, and who is passionate about supporting patients through all phases of health—including preventive care, pregnancy support, and family wellness.

Key Responsibilities

• Provide comprehensive primary care to patients across the lifespan
• Diagnose and manage acute and chronic conditions
• Perform wellness visits, physical exams, and preventive screenings
• Support women’s health and early prenatal care with education and monitoring
• Deliver pediatric care including well-child visits, immunizations, and same-day sick visits
• Conduct STI testing, counseling, and follow-up care
• Develop individualized care plans and coordinate referrals to specialists
• Educate patients on lifestyle, nutrition, and disease prevention
• Maintain accurate documentation in EHR systems

Qualifications

• Master’s or Doctorate in Nursing (FNP track)
• Board-certified Family Nurse Practitioner
• Active California RN & NP license
• Furnishing Number and DEA certification
• Strong clinical judgment and patient-centered communication skills

Preferred Experience

• 2+ years of outpatient primary care experience
• Experience in family medicine, pediatric and women’s health is a plus
• Familiarity with value-based care models and EHR systems
