Family Nurse Practitioner (FNP)

DocCare is an established primary care and multi-specialty medical practice with locations in Queens and Hauppauge, NY. We provide compassionate, high-quality healthcare to our patients. We are seeking a part time Family Nurse Practitioner (FNP) for approximately 30 hours per week with strong clinical skills and experience in primary care to join our Hauppauge, NY location. The ideal candidate will be comfortable working independently, managing a broad range of acute and chronic conditions, and collaborating with our medical and administrative teams to provide exceptional patient care. This position will rotate throughout our locations, as needed.

Responsibilities:

• Conduct wellness visits, sick visits, urgent care, and follow-up visits.
• Diagnose, develop treatment plans, and manage patient care effectively.
• Prescribe medications and monitor patient adherence and outcomes.
• Order and interpret diagnostic tests (EKG, PFT, ultrasound, x-rays, etc.).
• Administer vaccinations and preventive care services.
• Refer patients to specialists when necessary.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in clinical quality improvement programs to enhance patient care.
• Work collaboratively with the Medical Director, clinical staff, and administrative team to ensure smooth practice operations.

Minimum Requirements:

Must have an active NYS Registered Nurse (RN) license. Board Certification as a Family Nurse Practitioner. 3 to 5 years of direct patient care experience as a Nurse Practitioner necessary. Active New York State (NYS) Nurse Practitioner license required. Active DEA number needed. Active NPI number necessary. Malpractice insurance (covered by the company, if the individual doesn’t have their own).

Preferred Qualifications:

Prior experience in private practice or primary care settings is a plus. Certification to perform DOT physicals (or willingness to obtain certification within 90 days; company covers certification cost) strongly preferred.

Pay: $80.00 - $95.00 per hour
