Family Nurse Practitioner (FNP) - Part Time

Job Summary
We are seeking an entrepreneurial and clinically sharp Family Nurse Practitioner to join Luminum Health (Our new brand) as our founding clinical team member where you will co-design the clinical model from scratch, then serve as the first prescribing provider on the platform.

Luminum Health is an early-stage DTC telehealth startup delivering guided health programs not just prescriptions. Every member receives multiple layers of care: prescriptions shipped to door, AI-powered progress tracking, a 52-week education library, and private community. We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our healthcare team on a part-time basis. In this role, you will provide comprehensive primary care services, perform patient assessments, and deliver personalized treatment plans across diverse patient populations. Your expertise will help promote health, manage acute and chronic conditions, and support patients in achieving their wellness goals. You will operate as an independent clinical provider under an MSO/PC structure. You retain full independent clinical judgment over all prescribing and treatment decisions. Your first three months involve designing clinical protocols, building intake workflows, and stress-testing the platform alongside the founding team. Starting Month 4, you begin seeing real members. By Month 6, you help onboard and train other FNPs. At steady state, you manage approximately 350 members at 25 to 30 hours per month with AI or supporting staff handling 70% of all member interactions. 100% remote. 100% async. No video calls. No scheduled shifts.

Responsibilities

  • Co-design prescribing protocols, intake questionnaire logic, contraindication screening, and clinical escalation SOPs for four launch programs: hormonal skin/hair (Glow™), postpartum recovery with mood screening (Restore™), PCOS management with full lab panels (Thrive™), and weight/metabolic health (Reset™).
  • Operate as an independent clinical provider contracted through the Professional Corporation (PC) under Luminum’s MSO/PC structure, maintaining full autonomous clinical judgment over all prescribing, treatment, and patient care decisions without corporate direction or influence.
  • Review new member intake submissions asynchronously - health histories, photos, allergies, and screening forms and make independent prescribing decisions for FDA-approved, non-controlled generic medications including spironolactone, tretinoin, minoxidil, metformin, azelaic acid, bupropion/naltrexone, and topiramate.
  • Respond to clinical member messages routed to your queue by the AI classification system, or supporting staff including questions about side effects, symptoms, medication concerns, and treatment progress. All communication via HIPAA-compliant secure messaging - no video visits.
  • Conduct quarterly clinical reviews using AI-compiled 3-month member summaries (photos, labs, check-in history, adherence data) and make treatment adjustment decisions: continue, modify dosing, order labs, add medications, or refer out.
  • Administer and interpret PHQ-9/GAD-7 mood screening for postpartum (Restore™) members. Follow the crisis escalation SOP for scores indicating moderate-to-severe depression or suicidal ideation, including immediate outreach and 988 Lifeline referral when indicated.
  • Order and interpret laboratory results including BMP/potassium, A1C, fasting insulin, testosterone, DHEA-S, thyroid TSH, lipid panels, CMP, and liver function tests. Coordinate lab ordering through Quest Diagnostics and Labcorp.
  • Review monthly check-in messages for clinical accuracy and appropriateness.
  • Configure and test the clinical workspace on our technology platform including intake forms, prescribing templates, and pharmacy fulfillment pipeline. Run mock patients through the full flow before live launch.
  • Train and onboard other FNPs after launch on all clinical protocols, platform workflows, escalation procedures, and quality standards you helped design. Serve as the clinical lead who defines how every future Luminum provider practices.
  • Maintain active, unrestricted FNP license(s) in all operating states, current malpractice insurance ($1M/$3M minimum), active NPI. Comply with all HIPAA requirements.
  • Participate in weekly team syncs via MS Teams (and any other meetings required). Provide clinical input on marketing content compliance.

Requirements

  • Board-certified Family Nurse Practitioner (FNP-C) with active, unrestricted license. Multi-state NLC coverage strongly preferred.
  • Minimum 2 years of clinical experience as a prescribing NP in outpatient, primary care, urgent care, or telehealth settings.
  • Demonstrated prescribing experience with: spironolactone, tretinoin, metformin, minoxidil, bupropion/naltrexone, topiramate, or azelaic acid.
  • Comfortable operating under an MSO/PC structure as an independent contractor (1099) with full clinical autonomy. You understand the MSO handles business operations while you retain sole authority over all clinical decisions.
  • Experience with or willingness to work in fully asynchronous telehealth - all clinical communication via secure messaging with no video visits or scheduled appointments.
  • Active NPI number and current DEA registration (or ability to obtain). Luminum does not prescribe controlled substances but some pharmacy networks require DEA for credentialing.
  • Current professional liability / malpractice insurance with minimum $1M/$3M coverage, or willingness to obtain (Luminum assists with cost).
  • Proficiency with EHR systems and telehealth platforms. Experience with Bask Health, Healthie, DoseSpot, or similar DTC telehealth platforms is a plus.
  • Strong clinical documentation skills and ability to ensure encounters meet state-specific async telehealth requirements.
  • Comfortable with AI-assisted clinical workflows. Willingness to review, audit, and refine AI-generated patient communications.
  • Familiarity with HIPAA compliance, telehealth-specific informed consent, and state-by-state prescribing regulations for asynchronous care.
  • Clinical experience in hormonal and metabolic health - PCOS, postpartum, dermatologic conditions, insulin resistance, metabolic syndrome, or weight management.
  • Prior experience at a DTC telehealth company is preferred but not required (Hims, Ro, Nurx, Done, Calibrate, Allara, or similar). You understand async clinical workflows and high-volume queue management.
  • Startup mindset - energized by ambiguity, comfortable wearing multiple hats, want to build clinical systems rather than just work inside existing ones.
  • Experience with mood screening tools (PHQ-9, GAD-7) and postpartum mental health assessment.
  • Experience mentoring, training, or supervising other NPs or clinical staff. You see yourself growing into a clinical lead role.

Pay: $55.00 - $75.00 per hour
