Family Nurse Practitioner (FNP) – Primary Care

Job Summary
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) specializing in Primary Care to join our dedicated healthcare team. In this vital role, you will provide comprehensive, patient-centered care across diverse populations, including pediatrics, geriatrics, and adults. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions in a variety of clinical settings. Duties

  • Conduct thorough physical examinations and patient assessments to evaluate health status and identify medical issues.
  • Develop and implement individualized treatment plans, including medication administration, injections, suturing, and wound care.
  • Utilize EMR (Electronic Medical Record) systems like eClinicalWorks to document patient encounters accurately and efficiently.
  • Provide health coaching and education on disease prevention, medication adherence, nutrition, and lifestyle modifications.
  • Coordinate care with specialists in areas such as gastroenterology, infectious disease, dermatology, and hospice & palliative medicine.
  • Participate in triage activities via telehealth or in-person visits to prioritize urgent cases and ensure timely patient care.
  • Maintain compliance with HIPAA regulations and ensure the confidentiality of all patient records.

Qualifications

  • Valid Family Nurse Practitioner (FNP) certification with current state licensure.
  • Proven experience in primary care settings with proficiency in medical office procedures and EMR/EHR systems.
  • Demonstrated knowledge of clinical research methodologies and familiarity with CPT coding, ICD-10 coding standards.
  • Experience working with diverse populations including pediatrics, geriatrics, assisted living residents, nursing home patients, or individuals with developmental disabilities.
  • Skills in performing procedures such as suturing, phlebotomy, venipuncture, catheter care, sterile processing techniques, and basic life support certification.
  • Strong understanding of physiology, anatomy knowledge, diagnostic evaluation techniques including sonography or laboratory experience preferred.
  • Ability to handle complex cases involving acute pain management, infectious disease care, trauma (Level I/II trauma center experience), or post-acute care scenarios like hospice or memory care.
  • Excellent communication skills for patient education and collaboration with multidisciplinary teams across inpatient or outpatient settings. Pay: From $85,000.00 per year
