Family Nurse Practitioner or Physician Assistant

Overview
Join our dynamic healthcare team as a Family Nurse Practitioner or Physician Assistant, where your expertise will directly impact patient care across diverse clinical settings. In this role, you will provide comprehensive primary, urgent and specialty care, utilizing your advanced clinical skills to assess, diagnose, and manage a wide range of health conditions. Responsibilities

  • Conduct thorough patient assessments, physical examinations, and medical history reviews to determine health status and needs.
  • Diagnose acute and chronic illnesses across various specialties such as geriatrics, pediatrics, behavioral health, and urgent care /emergency medicine.
  • Develop and implement individualized treatment plans incorporating medication management, procedures like suturing and IV infusion, and non-pharmacologic therapies.
  • Utilize electronic health record (EHR) systems such as Athena or eClinicalWorks for accurate documentation of patient encounters, medical records, coding ICD-10, CPT coding, and billing processes.
  • Coordinate care with specialists in areas like gastroenterology, infectious disease care, hospice & palliative medicine, and occupational health to ensure comprehensive patient management.
  • Provide telehealth services leveraging telemedicine platforms to expand access to care
  • Engage in patient education on topics such as family planning, nutrition, memory care, and disease prevention strategies tailored to individual needs.

Experience

  • Proven clinical experience working in Primary Care, Urgent Care and/or Internal Medicine.
  • Hands-on experience with procedures including sutures, IVs, Joint Injections, I and D, and Botox
  • Demonstrated ability to manage diverse patient populations including pediatrics , geriatrics (memory care), assisted living residents, and individuals with developmental disabilities or dementia.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, eClinical Works or Athenahealth for efficient documentation and order entry.
  • Experience performing triage assessments in urgent care or emergency room settings with acute care skills.
  • Knowledge of Workers' Compensation law, DOT requirements and utilization review processes for occupational health cases.
  • Skilled in performing sterile techniques for procedures like phlebotomy, IVs, specimen collection/processing, catheter care, and sterile processing protocols.
