Job Summary
We are actively seeking a dedicated and highly skilled Family Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team. The ideal candidate will possess a comprehensive clinical background, demonstrating expertise in primary and specialized care across diverse patient populations, including pediatrics, geriatrics, and acute care settings. This role offers an opportunity to deliver high-quality patient-centered services, utilizing advanced clinical skills and a broad spectrum of medical procedures. The successful applicant will be instrumental in providing comprehensive assessments, managing complex cases, and supporting continuity of care within a dynamic healthcare environment.
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform diagnosis and treatment plans.
- Manage acute and chronic conditions utilizing evidence-based practices, including medication administration, IV infusion therapy, suturing, and wound care.
- Coordinate with multidisciplinary teams to develop discharge planning, case management strategies, and post-acute care plans tailored to individual patient needs.
- Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks for accurate documentation of patient encounters and medical records management.
- Engage in telehealth consultations to expand access to care while maintaining compliance with HIPAA regulations.
- Provide education on health coaching topics including nutrition, family planning, memory care, and senior or hospice & palliative medicine support.
Requirements
- Valid licensure as a Family Nurse Practitioner or Physician Assistant with current certification from recognized licensing boards.
- Demonstrated experience working in diverse healthcare settings such as hospitals (Level I & II trauma centers), emergency rooms, ICU/PICU units, nursing homes, assisted living facilities, or outpatient clinics.
- Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for efficient documentation and patient management.
- Strong clinical skills encompassing physical examinations, diagnostic evaluation, specimen collection/processing, and medication administration.
- Experience with advanced procedures such as IV insertion/venipuncture, sterile technique for suturing or dermal fillers, airway management, and gastrostomy tube feeding is preferred.
- Knowledge of CPT coding (Current Procedural Terminology), ICD-10/ICD-9 coding systems for accurate billing and documentation purposes.
- Familiarity with specialties including geriatrics, pediatrics (including toddler and neonatal care), behavioral health, occupational health, hospice & palliative medicine, and infectious disease management.
- Ability to work effectively in fast-paced environments requiring triage skills and acute pain management expertise.
- Excellent communication skills with the ability to collaborate across multidisciplinary teams while maintaining patient confidentiality per HIPAA standards. Pay: $90,000.00 - $120,000.00 per year
