Family Outreach Caseworker

Casakids is seeking a compassionate, organized, and trauma-informed Family Outreach Case Worker to join our Family Resource Center team. This position provides direct support to families through in-home and community-based services designed to strengthen family stability, reduce the likelihood of repeat maltreatment, and connect families to meaningful resources and support.

This role is ideal for someone who is passionate about working with families, values relationship-based support, and can provide practical, hands-on guidance with professionalism, empathy, and accountability.

Position Summary

The Family Outreach Case Worker is responsible for delivering prevention-based, differential response services to families referred through the Family Outreach Program. Families are referred following a screened-out CYFD report that does not meet the statutory definition of abuse or neglect but still reflects a need for support, guidance, and connection to services.

This position provides in-home and community-based case management, assessments, service planning, referrals, support, and follow-up services. The Family Outreach Case Worker must maintain a high level of ethics, confidentiality, trauma-informed practice, and professional judgment at all times.

Essential Duties and Responsibilities

  • Provide in-home and community-based family support services in accordance with program requirements
  • Review referrals and supporting documentation received from CYFD Family Outreach staff
  • Participate in warm handoff meetings with families, referral sources, and support persons
  • Complete intake and enrollment documentation with families
  • Build rapport and engage families in services using a trauma-informed, strengths-based approach
  • Conduct screenings, assessments, informal safety observations, and service planning
  • Provide case management, referrals, resource linkage, and hands-on family support
  • Support families through a tiered case management model from intake through discharge
  • Assess family readiness, engagement, and progress throughout service delivery
  • Maintain timely, accurate, and complete documentation in the case record
  • Complete progress notes, case staffing documentation, discharge summaries, surveys, and related records
  • Track required performance measures and ensure data is entered accurately
  • Collaborate with community providers and partner agencies to avoid duplication of services
  • Participate in required meetings, trainings, and supervision
  • Observe and report safety concerns and follow all mandatory reporting requirements
  • Maintain coverage of work responsibilities during absences
  • Arrive on time to meetings, appointments, and scheduled visits
  • Perform other duties as assigned

Qualifications:

Bilingual (Spanish/English) preferred but not required.

Education

  • Bachelor’s degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, Family Services, or another related field approved by CYFD.

OR

  • A combination of related education and direct experience totaling four (4) years

Pay: $40,000.00 - $45,000.00 per year
