Family Practice/Primary Care Nurse Practitioner

Job Title- Nurse Practitioner / Physician Assistant - Charlotte, NC
Shift - Days
Type - Full Time
Schedule: Monday - Friday, 8:00 AM - 5:00 PM

Key Responsibilities:-

• Patient Care: Perform comprehensive assessments, diagnose, and manage acute and chronic medical conditions while providing high-quality, evidence-based care.
• Clinical Procedures: Conduct procedures within the scope of practice and clinical protocols.
• Collaborative Practice: Work closely with physicians, nurses, and multidisciplinary teams to ensure coordinated and efficient patient care.
• Chronic Disease Management: Provide ongoing care, monitoring, and education for patients with chronic illnesses to improve health outcomes.
• Health Promotion: Participate in health promotion, disease prevention, and patient education initiatives.
• Documentation: Maintain accurate, timely, and confidential electronic health records (EHR).
• Regulatory Compliance: Ensure adherence to federal, state, and facility standards and regulations.
  

Qualifications:-

• Current and unrestricted North Carolina Nurse Practitioner (NP) or Physician Assistant (PA) license (or eligibility to obtain prior to start date).
• National board certification (AANP, ANCC, or NCCPA).
• Minimum of 2 years of clinical experience (preferably in Primary Care or Occupational Health).
• BLS and ACLS certifications required.
• Proficiency with Electronic Health Record (EHR) systems.
• Strong clinical judgment, diagnostic, and communication skills.
• Compassionate, patient-focused, and culturally sensitive approach to care.
• Exposure to Medication-Assisted Treatment (MAT) preferred but not required.
